目的:探讨类风湿性关节炎(RA)患者血清透明质酸(HA)及其相关蛋白(SHAP)水平的变化.方法:采用酶联免疫法对采集的114例RA患者的血清进行HA和SHAP浓度测定,并以31名正常血清作对照;同时应用免疫组织化学法检测HA和SHAP在RA患者滑膜和软骨中的表达.结果:RA组血清HA和SHAP浓度明显高于正常组,RA组血清HA、 SHAP水平与正常对照组相比差异均有统计学意义,RA组血清HA和SHAP浓度两者有显著相关性;除血清SHAP与血沉呈正相关且有显著性外,血清HA与血沉及血清C-反应蛋白均无相关性.SHAP-HA在RA患者滑膜和软骨中的表达明显低于HA.结论:RA患者血清HA和SHAP-HA浓度增加是RA相对独立的炎症反应指标.RA发展中血清SHAP-HA更有监测意义.

Objective: To investigate the changes and clinical significance of hyaluronan (HA) and hyaluronan-associated proteins (SHAP) level in serum in patients with rheumatoid arthritis (RA). Methods: The HA and SHAP level of serum was measured by enzyme-linked immmunosorbent assay (ELISA) in 114 patients with RA and 31 normal serum samples from healthy donors. An immunohistochemical technique was used to detect the expression of HA and SHAP in the cartilage and synovium of RA patients. Results: The HA and SHAP levels of serum in RA patients were significantly increased compared with normal serum (P<0.01). There were significant positive correlations between the levels of SHAP and HA (r=0.856, P<0.01), and positive correlations between SHAP and ESR in patients with RA. However, HA was not closely correlated with ESR and CRP. Compared with the distribution pattern of HA, that of SHAP was patchier. Conclusion: Serum HA and SHAP may be unique as an inflammatory marker of RA. The SHAP levels relate more closely than the HA levels to the inflammation progress in patients with RA.
