目的 探讨醒脑静注射液在抢救重度乙醇中毒中的作用.方法 将133例在我院急诊科抢救的重度乙醇中毒患者随机分为治疗组61例(醒脑静注射液联合纳洛酮治疗)和对照组72例(单一纳洛酮治疗),监测并记录呼吸、收缩压、体温恢复时间,清醒时间,治疗1、4、8 h后末梢血氧饱和度(SpO_2)、氧分压(PaO_2)、氧合指数和二氧化碳分压(PaCO_2).结果 治疗组呼吸、收缩压、体温恢复和清醒时间均较对照组短,差异有统计学意义(P<0.05).治疗1 h后,治疗组SpO_2开始升高,与对照组比较差异有统计学意义(P<0.05);治疗4 h后,治疗组SpO_2、PaO_2、氧合指数全面升高,PaCO_2下降,与对照组比较差异有统计学意义(P<0.05);治疗8 h后,两组SpO_2、PaO_2、氧合指数、PaCO_2变化间差异无统计学意义(P>0.05).结论 醒脑静注射液联合纳洛酮抢救重度乙醇中毒可有效缓解患者中毒症状,明显缩短清醒时间,改善机体氧合能力,疗效比单用纳洛酮迅速、确切,可以在临床急救中推广应用.

목적 탐토성뇌정주사액재창구중도을순중독중적작용.방법 장133례재아원급진과창구적중도을순중독환자수궤분위치료조61례(성뇌정주사액연합납락동치료)화대조조72례(단일납락동치료),감측병기록호흡、수축압、체온회복시간,청성시간,치료1、4、8 h후말소혈양포화도(SpO_2)、양분압(PaO_2)、양합지수화이양화탄분압(PaCO_2).결과 치료조호흡、수축압、체온회복화청성시간균교대조조단,차이유통계학의의(P<0.05).치료1 h후,치료조SpO_2개시승고,여대조조비교차이유통계학의의(P<0.05);치료4 h후,치료조SpO_2、PaO_2、양합지수전면승고,PaCO_2하강,여대조조비교차이유통계학의의(P<0.05);치료8 h후,량조SpO_2、PaO_2、양합지수、PaCO_2변화간차이무통계학의의(P>0.05).결론 성뇌정주사액연합납락동창구중도을순중독가유효완해환자중독증상,명현축단청성시간,개선궤체양합능력,료효비단용납락동신속、학절,가이재림상급구중추엄응용.
Objective To discuss the effect of Xing-nao-jing injection in rescuing severe ethylism.Methods Totally 133 patients with severe ethylism undergoing emergent aid in our hospital were randomly divided as trial group (n=61) and control group (n=72).The patients in the trial group were treated with Xing-nao-jing injection and naloxone hydrochloride,while those in the control group were treated with naloxone hydrochloride only.The recovery times of respiration,systolic blood pressure,body temperature,and consciousness,as well as the levels of SpO_2,PaO_2,oxygenation index (PaO_2/FiO_2),and PaCO_2 1,4 and 8hour after treatment were monitored.Results The recovery times of respiration,systolic blood pressure,body temperature and consciousness in the trial group were shorter than those in the control group (P<0.05).Compared with the control group,one hour later the SpO_2 level began to increase in the trial group (P<0.05);four hours later,the levels of SpO_2,PaO_2,and PaO_2/FiO_2 increased,and the level of PaCO_2 decreased.However eight hours later,there was no significant difference between the two groups (P>0.05).Conclusion Xing-nao-jing injection combined with naloxone hydrochloride could relieve toxic symptom efficiently,shorten the conscious recovery time obviously,and improve organism oxygenic capability in rescuing severe ethylism,which is of more rapid and certain effect than naloxone hydrochloride alone,can be extensively applied in clinical emergency aid.
