目的 探讨Th17细胞和Treg细胞失衡在特应性皮炎(AD)发病机制中的作用.方法 流式细胞仪检测52例AD患者外周血Th17细胞和Treg细胞的频率、酶联免疫吸附方法(ELISA)检测外周血中细胞因子IL-6、TGF-β1的表达水平.同时以30例性别、年龄匹配的健康体检者作为对照.结果 AD组外周血Th17细胞(CD3+CD8-IL17+)占CD33+细胞的百分比为(1.20±0.41)%,高于对照组的(0.54±0.28)%(f=2.58,P<0.05);Treg(CD4+CD25+)细胞的百分比为(2.29±0.67)%,低于对照组(5.95±0.45)%,(t=15.23,P< 0.01).关键调控因子测定结果:IL-6水平,AD组(5.12±0.45)ng/L高于对照组(3.89±0.38) ng/L,差异具有统计学意义(t=2.59,P<0.05);而TGF-β1的表达水平AD组(57.65±10.78) ng/L低于对照组的(81.18±7.78) ng/L,(t=5.41,P< 0.01).结论 特应性皮炎患儿外周血Th17、Treg细胞水平及其关键的调控平衡因子IL-6、TGF-β1发生变化,其比例的失平衡可能参与特应性皮炎的发病.

목적 탐토Th17세포화Treg세포실형재특응성피염(AD)발병궤제중적작용.방법 류식세포의검측52례AD환자외주혈Th17세포화Treg세포적빈솔、매련면역흡부방법(ELISA)검측외주혈중세포인자IL-6、TGF-β1적표체수평.동시이30례성별、년령필배적건강체검자작위대조.결과 AD조외주혈Th17세포(CD3+CD8-IL17+)점CD33+세포적백분비위(1.20±0.41)%,고우대조조적(0.54±0.28)%(f=2.58,P<0.05);Treg(CD4+CD25+)세포적백분비위(2.29±0.67)%,저우대조조(5.95±0.45)%,(t=15.23,P< 0.01).관건조공인자측정결과:IL-6수평,AD조(5.12±0.45)ng/L고우대조조(3.89±0.38) ng/L,차이구유통계학의의(t=2.59,P<0.05);이TGF-β1적표체수평AD조(57.65±10.78) ng/L저우대조조적(81.18±7.78) ng/L,(t=5.41,P< 0.01).결론 특응성피염환인외주혈Th17、Treg세포수평급기관건적조공평형인자IL-6、TGF-β1발생변화,기비례적실평형가능삼여특응성피염적발병.
Objective To evaluate the roles of imbalance between peripheral blood T helper 17 (Th17) cells and CD4+CD25+ regulatory T (Treg) cells in the pathogenesis of atopic dermatitis (AD).Methods Peripheral blood samples were obtained from 52 patients with AD aged 2-14 years and 30 age- and sex-matched healthy controls.Flow cytometry was performed to detect the percentage of Th17 cells and Treg cells in peripheral blood.Meanwhile,enzyme linked immunosorbent assay (ELISA) was carried out to detect the serum level of interleukin (IL)-6 and transforming growth factor (TGF)-β1.Results The children with AD showed a higher percentage of Th17 cells but a lower percentage of Treg cells in CD3+ T cells compared with the controls (( 1.20 ± 0.41 )% vs.(0.54 ± 0.28)%,t =2.58,P < 0.05; (2.29 ± 0.67)% vs.(5.95 ± 0.45)%,t =15.23,P < 0.01 ).Moreover,the serum level of IL-6 was significantly higher,while that of TGF-β1 was lower in patients with AD than in the controls ((5.12 ± 0.45) ng/L vs.(3.89 ± 0.38) ng/L,t =2.59,P< 0.05; (57.65 ± 10.78) ng/L vs. (81.18 ± 7.78) ng/L,t =5.41,P < 0.01 ).Conclusions Children with AD experience a change in the percentage of Thl7 cells and Treg cells in peripheral blood as well as in the serum level of IL-6 and TGF-β1,and the imbalance between Th17 cells and Treg cells in peripheral blood may contribute to the development of AD.
