目的 探讨建立大鼠移植心脏心肌内心电图(IMEG)描记模型的方法.方法 套管法建立大鼠颈部异位心脏移植.应用150 mm长、直径为0.09 mm的漆包线作为心肌内电极,采用穿刺法分别于右室前壁、心尖部及左室游离壁置入心肌内电极,并将3个电极从大鼠背部对应位置引出,固定,记录各电极在心肌内位置.结果实验大鼠60只,53只成功,成功率88.3%.7只因心肌内电极置入处出血而造模失败.手术时间为(71±6)min,其中心脏移植手术时间为(57±5)min,心肌内电极置入时间(9±2)min.移植心脏存活期间,49只(92.5%)可正常描记心肌内心电图至排斥终末.4只失败的原因:电极断裂1只,电极传导能力下降3只.结论 穿刺法建立大鼠移植心脏IMEG描记模型,将心肌内电极置入过程简化,术后存活率明显提高,易于重复,是一种研究心脏移植后IMEG较理想的动物模型.

Objective To seek a simplified technique for rat intramyocardial electrocardiogram (IMEG) model.Methods The rat heterotopic neck heart transplantation technique used has been thoroughly described previously.A 150-mm long enamelled copper wire with a diameter of 0.09 mm was used as an intramyocardial electrode.Each end of enamelled copper wire was burnt to eliminate 4 mm long enamel out of copper wire.Prepared enamelled copper wire was led through a 26 G infusion needle and was everted over cusp about 1 mm.After having finished preparation of recipient and donor heart,the infusion needle with intramyocardial electrode was inserted into the right ventricle lack of coronary vessels.Infusion needle was gently swivelled and pulled out of the heart.Intramyocardial electrode was left inside myocardium.Medical glue was daubed around the electrode.Other two intramyocardial electrodes were inserted into left ventricle and cardiac apex of the allograft using the same method described above.Results IMEG monitoring could be performed successfully as long as the donor heart beating.The successful rate of 60 formal experiments was 88.3%.The time of procedure was (71 ±6) min.Complications such as inserted point hemorrhage of intramyocardial electrode were abated distinctly. The postoperative survival rate was increased.Conclusion Rat IMEG model is an ideal IMEG research model for its cheap price which avails to mass experiments.More importantly,the whole donor heart is cut out to have myocardial pathological evaluation which could avoid of sampling error of endomyocardial biopsy.
