目的 探讨低分子右旋糖酐和丹参注射液对经皮冠脉介入(PCI)治疗后造影剂肾损伤的保护作用和预防肾功能损伤的效果.方法 将行PCI术的120例患者随机分为低分子右旋糖酐和丹参注射液治疗组与对照组各60例,治疗组在常规治疗基础上加用低分子右旋糖酐250 ml和丹参注射液16 ml应用7d.两组均在治疗前及术后第1天、第6天检测血尿素氮、肌酐、β2微球蛋白及24h蛋白尿.结果 两组造影剂应用后第1天血尿素氮、肌酐、β2微球蛋白及24h蛋白尿均升高,第6天恢复至基线水平,治疗组明显低于对照组(均P<0.05).结论 围手术期应用低分子右旋糖酐和丹参注射液治疗可有效减轻造影剂对肾脏的损害,减低肾功能不全的发生率.

목적 탐토저분자우선당항화단삼주사액대경피관맥개입(PCI)치료후조영제신손상적보호작용화예방신공능손상적효과.방법 장행PCI술적120례환자수궤분위저분자우선당항화단삼주사액치료조여대조조각60례,치료조재상규치료기출상가용저분자우선당항250 ml화단삼주사액16 ml응용7d.량조균재치료전급술후제1천、제6천검측혈뇨소담、기항、β2미구단백급24h단백뇨.결과 량조조영제응용후제1천혈뇨소담、기항、β2미구단백급24h단백뇨균승고,제6천회복지기선수평,치료조명현저우대조조(균P<0.05).결론 위수술기응용저분자우선당항화단삼주사액치료가유효감경조영제대신장적손해,감저신공능불전적발생솔.
Objective To explore the protective effect of low molecular dextran combined with salvia miltiorrhiza injection against kidney damage from contrast agent after percutaneous coronary intervention(PCI) and the effect of preventing kidney damage.Methods 120 patients who underwent PCI were randomly divided into two groups:the treatment group( low molecular dextran combined with salvia miltiorrhiza treatment) and the control group,each group 60 cases.The control group was given the conventional treatment,and the treatment group was treated with 250ml low molecular dextran and 16ml salvia miltiorrhiza injection for 7d on the basis of conventional treatment.The levels of blood urea nitrogen ( BUN ),serum creatinine ( SCR),β2 microglobulin ( β2-MG),24h urine protein were detected before and 1 d,6d after surgery.Results At one day after application of contrast agent,the levels of BUN,SCR,β2-MG,24h urine protein were increased,and returned to baseline level at 6th day.The levels of BUN,SCR,β2-MG,24h urine protein of the treatment group were significantly lower than those of the control group at 6th day ( P < 0.05 ).Conclusion The low molecular dextran combined with danshen injection treatment in the perioperative period could effectively reduce the kidney impairment from contrast agent and the incidence of renal insufficiency.
