目的 探讨外源性重组人骨形态发生蛋白-2(rhBMP-2)应用于兔腰椎后路横突植骨融合中的促成骨效应细胞增殖作用及其成骨机制. 方法 45只新西兰大白兔随机分为三组(n=15),建立腰椎后路横突间植骨融合模型,分别植入rhBMP-2/异体骨复合骨条(复合骨组)、自体髂骨条(自体骨组)、单纯异体髂骨条(异体骨组).用流式细胞仪检测2、7、14、28、35 d具有成骨效应的骨髓基质细胞(MSCs)、成骨细胞、血管内皮细胞的增殖量. 结果复合骨组MSCs增殖量在术后2、7、35 d均比自体骨组和异体骨组高,差异均有统计学意义(P<0.05).复合骨组成骨细胞增殖量除在术后2 d高于自体骨组,差异有统计学意义(P=0.028)外,在其他时间点差异均无统计学意义(P>0.05),但复合骨组成骨细胞增殖量在术后2、7、14、28、35 d时均高于异体骨组,差异有统计学意义(P<0.05).复合骨组血管内皮细胞增殖量在术后2、7、28 d均高于自体骨组和异体骨组,差异有统计学意义(P<0.05).结论 在脊柱融合的不同时间段,外源性rhBMP-2能有效地促进MSCs、成骨细胞、血管内皮细胞增殖.

목적 탐토외원성중조인골형태발생단백-2(rhBMP-2)응용우토요추후로횡돌식골융합중적촉성골효응세포증식작용급기성골궤제. 방법 45지신서란대백토수궤분위삼조(n=15),건립요추후로횡돌간식골융합모형,분별식입rhBMP-2/이체골복합골조(복합골조)、자체가골조(자체골조)、단순이체가골조(이체골조).용류식세포의검측2、7、14、28、35 d구유성골효응적골수기질세포(MSCs)、성골세포、혈관내피세포적증식량. 결과복합골조MSCs증식량재술후2、7、35 d균비자체골조화이체골조고,차이균유통계학의의(P<0.05).복합골조성골세포증식량제재술후2 d고우자체골조,차이유통계학의의(P=0.028)외,재기타시간점차이균무통계학의의(P>0.05),단복합골조성골세포증식량재술후2、7、14、28、35 d시균고우이체골조,차이유통계학의의(P<0.05).복합골조혈관내피세포증식량재술후2、7、28 d균고우자체골조화이체골조,차이유통계학의의(P<0.05).결론 재척주융합적불동시간단,외원성rhBMP-2능유효지촉진MSCs、성골세포、혈관내피세포증식.
Objective To explore the effects of exogenous recombined human bone morphogenetic protein-2(rhBMP-2)on cell proliferation in posterolateral lumbar spinal fusion in a rabbit model. Methods Forty-five rabbits that had received posterior graft fusion of transverse process of lumbar vertebra were randomly divided into 3 groups: rhBMP-2/allograft composite group, autologous iliac crest bone graft group, and allograft group. The proliferations of marrow stromal cells (MSCs), osteoblasts and vascular endothelial cells were detected at different times by flow cytometry. Results The numbers of MSCs in the rhBMP-2/allograft composite group at the 2nd, 7th, 35th days after graft transplantation were significantly greater than in the autologous iliac crest bone graft group and allograft group (P<0.05). There were significant differ-ences in the quantity of osteablasts at the 2nd, 7th, 14th, 28th, 35th days after graft transplantation in all 3 groups (P<0.05), but there was no significant difference in the quantity of osteoblasts at the 7th, 14th, 28th, 35th days after operation between the composite group and autologous iliac crest bone graft group (P>0.05) except at the 2nd day (P=0.028). The quantity of osteoblasts in composite group was significantly larger than in the allograft group at all time points (P<0.05) . The quantities of vascular endothelial cells in the composite group at the 2nd, 7th, 28th days after operation were significantly larger than in the autologous iliac crest bonegraft group (P<0.05). Conclusion The exogenous rhBMP-2 may have a promoting effect on proliferation of MSCs, osteoblasts, vascular endothelial cells at different times during the spinal graft fusion.
