目的 探讨川崎病患儿血脂变化与冠状动脉病变的关系.方法 采用氧化酶法和GPO-POD酶法分别测定46例川崎病患儿及30例健康儿童的血三酰甘油(TG)、胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)和高密度脂蛋白(HDL-C).结果 川崎病患儿急性期治疗前血TG、TC及LDL-C增高,与正常对照组间差异有统计学意义(P<0.05),血HDL-C降低,与正常对照组间差异有统计学意义(P<0.05).治疗后未并发冠脉病变组TG、TC及LDL - C均较治疗前降低,与治疗前相比差异有统计学意义(P<0.05),HDL-C较治疗前升高,与治疗前相比差异有统计学意义(P<0.01),与正常对照组相比差异无统计学意义(P>0.05);而并发冠状动脉病变组血脂各指标与治疗前比较差异无统计学意义(P>0.05).结论 血脂的动态测定可预测川崎病患儿并发冠状动脉病变的发生,有助于早期诊断.

Objective To study the relationship between the changes of serum lipid and the pathological changes of coronary artery in children with Kawasaki disease(KD).Methods The levels of serum TG,TC,LDL-C and HDL-C were determined by methods of oxidase and GPO-POD in 46 KD and 30 healthy children.Results The levels of TG,TC,LDL-C increased and HDL-C decreased in KD children in acute phase before treatment,significantly different from those in control group (P<0.05).TG,TC,LDL-C decreased (P<0.05),and HDL-C increased(P<0.01) in group without coronary artery lesion(CAL) after treatment as compared with pretreatment,not significantly different from normal control group(P>0.05).No significant difference was noted in various indicators of blood lipid between pre- and post-treatments in CAL group(P>0.05).Conclusion Dynamic determination of blood lipid,which can predict the occurrence of KD with CAL,is helpful in its early diagnoses.
