目的 研究胆道闭锁(BA)患儿肝脏组织中上皮-间充质转化过程中特异性细胞因子的表达及胚胎期发育相关基因的表达激活.旨在探讨BA肝脏纤维化等系列改变的分子机制.方法 取经手术证实为BA患儿的8例肝活检组织(年龄2~3个月),以无消化道疾病尸检婴儿8例肝脏组织做正常对照(年龄0~2个月),RT-PCR半定量法检测胆管上皮细胞特异性因子CK19,纤维化早期指标Ⅰ型胶原蛋白COL1A1,发育相关基因Notch基因受体HES1,TGF-β及其阳性信号分子Smad3的表达水平变化.结果 BA患儿肝脏组织中CK-19,COL1A1、HES1、TGF-β及Smad3等基因表达水平均明显提高,实验组及对照组差异显著.结论 BA患儿肝脏组织胶原组织增生,同时有胆管上皮细胞增生,胚胎期肝脏组织发育相关基因Notch信号通路系统重新激活,TGF-β及其阳性信号分子Smad3在这一过程中发挥作用.

목적 연구담도폐쇄(BA)환인간장조직중상피-간충질전화과정중특이성세포인자적표체급배태기발육상관기인적표체격활.지재탐토BA간장섬유화등계렬개변적분자궤제.방법 취경수술증실위BA환인적8례간활검조직(년령2~3개월),이무소화도질병시검영인8례간장조직주정상대조(년령0~2개월),RT-PCR반정량법검측담관상피세포특이성인자CK19,섬유화조기지표Ⅰ형효원단백COL1A1,발육상관기인Notch기인수체HES1,TGF-β급기양성신호분자Smad3적표체수평변화.결과 BA환인간장조직중CK-19,COL1A1、HES1、TGF-β급Smad3등기인표체수평균명현제고,실험조급대조조차이현저.결론 BA환인간장조직효원조직증생,동시유담관상피세포증생,배태기간장조직발육상관기인Notch신호통로계통중신격활,TGF-β급기양성신호분자Smad3재저일과정중발휘작용.
Objective To study the expression of special cytokines and related genes in patients with biliary atresia.Methods Liver biopsy specimen from 8 infants(age 1-3 months)with biliary atresia and liver tissue from 8 infants(age 1-3 months)without digestive disease were studieel.RT-PCR of epithelium specific cytokine CK19,early fibrosis factor COLAl,developmental gene Notch re-ceptor HES1,TGF-β and positive receptor Smad3 were performed.Results The expressions of CK-19,COLA1,HES1,TGF-β,Smad3 were all up-regulated in liver tissues from biliary atresia patients.The differences were significant.Conclusions In livers of biliary atresia,there's proliferation of bile duct epithelium.The collagen gene CLA1,developmental gene Notch,TGF-β and its transducer,Smad3 were up-regulated.This suggests a possihlc role for these genes.
