目的 分析维持性透析患者血液透析过程中发生低血压的原因及预防措施.方法 回顾性分析了河南省郸城县人民医院透析科2008年1月至2009年1月终末期肾脏病患者164例次(16例)发生透析相关性低血压的临床资料.结果 16例患者(164例次)在透析相关性低血压中单次超滤量超过干体重5%的患者共发生透析相关性低血压118例次(72.0%);单次超滤量<5%的患者发生透析相关性低血压46例次(28.0%);60岁以上的老年透析患者共发生透析相关性低血压93例次(56.7%),60岁以下的老年透析患者共发生透析相关性低血压71例次(43.3%).结论 透析间期容量负荷增长过多及高龄是发生透析相关性低血压的主要原因.控制容量负荷有可能减少透析相关性低血压的发生.

Objective To analyse the cause and precautionary measures of hypoteasion during the hemodialysis in patients with end stage renal disease (ESRD) .Methods A cohort of 16 ESRD patients with 164 hemodialysis related hypoteasion in the People's hospital of Dancheng country during January, 2008 to January, 2009 were studied. The clinical data were collected. Results Among the 16 ESRD patients with 164 hemodialysis related hypotension, 118 hypotension was more uhrafiltration, whose ultrafihration was over 5% dry weight, the prevalence was 72.0%. 93 hypotension during hemodialysis was normal ultrafiltration, whose ultrafiltration was less 5% dry weight, the prevalence was 28.0%. 93 hemodialysis related hypotension were in old patients whose age were more than 60 years old, the prevalence was 56.7%. 71 he-modialysis related hypotension were in patients whose age were less than 60 years old, the prevalence was 43.3 %. Conclusions Volume overload and eld were the main causes of hemodialysis related hypotension. So volume control maybe reduce the prevalence of hypotension during hemodialysis.
