目的探讨分析双水平气道正压(BiPAP)无创通气治疗慢性阻塞性肺疾病(COPD)并发Ⅱ型呼吸衰竭的临床疗效.方法选取我院于2010年1月至2011年12月间收治的72例慢性阻塞性肺疾病并发Ⅱ型呼吸衰竭的患者,随机分为对照组与实验组各36例,对照组接受传统治疗,实验组接受与对照组完全相同的传统治疗,另接受双水平气道正压无创通气治疗,观察其临床疗效.结果经过治疗后实验组PH,氧分压、二氧化碳分压,呼吸频率,心律,平均住院时间明显优于对照组,2组患者的差值比较差异有显著性(P＜0.05).结论双水平气道正压无创通气治疗慢性阻塞性肺疾病并发Ⅱ型呼吸衰竭有良好的临床疗效,值得推广.

Objective To study the clinical curative effect of BiPAP noninvasive ventilation in treatment of chronic obstructive pulrnonary disease (COPD) with type Ⅱ respiratory failure.Methods 72 patients with COPD with type Ⅱ respiratory failure accepted in our hospital from January 2010 to December 2011 were randomly divided into the experimental group and the control group.The control group with 36 patients was treated with the traditional treatment.The experimental group was treated with BiPAP noninvasive ventilation based on the traditional treatment.We observed the clinical curative effect of the 2 groups.Results After the treatment,the blood PH,PaO2,PaCO2,respiratory rate,heart rate and the average hospitalization time of the experimental group were obviously better than those of the control group.The difference between the control and the experimental group was statistically significant(P ＜ 0.05).Conclusions The clinical curative effect of BiPAP noninvasive ventilation in treatment of COPD with type Ⅱ respiratory failure has a significant effect and it is worth promoting.
