目的提高对后颅凹非脑组织的单发占位性病变MRI诊断的准确性.方法回顾性分析我院经手术、病理证实的占住性病变20例,复习MRI表现,不同病变的MRI具有的特征与共性表现.结果小脑半球转移瘤7例(误诊2例),听神经瘤5例,脑膜瘤6例(误诊2例),小脑脓肿2例(1例误诊).定性正确诊断率75%(15/20),共性表现为病灶T1WI等低信号,T2WI等高信号,呈结节形、囊+壁结节形、环形强化.结论性质不同病变有相似的MRI表现,也有特征性的表现,结合年龄、发病部位及形态学表现MRI可作出准确诊断.

Objective To improve the MRI diagnostic accuracy of non-cerebellar occupying lesions in pos-terior fossa.Methods MRI manifestations of posterior fossa occupying lesions proved by operation and pathology in 20 cases were retrospectively analyzed.MRI features had specific and common characteristics.Results There was cerebellar metastatic tumor in 7 cases including misdiagnosis in 2 cases,neurilemoma of acoustic nerve in 5 cases,meningoma in 6 cases including misdiagnosis in 2 cases,and abscess in 2 cases including misdiagnosis in one case.The qualitative accuracy of MRI diagnosis was 75%,the common features included isointensity or hypo-isointensity on T1W1,isointencity or hyperintencity on T2W1,nodle,cyst and wall nodle or circularity after contrast-enhance-ment.Conclusion Different lesions have specific and common characteristics or MRI,which can contribute to ac-curate diagnosis for the single occupying lesion combining with MRI manifestation and patients age and mass posi-tion.
