目的 探讨长期无创正压通气在稳定期慢性阻塞性肺疾病(COPD)患者中的应用价值.方法 将符合条件的54例稳定期重度COPD患者分为治疗组和对照组,每组各27例,治疗组在一般治疗的同时给予长期无创正压通气治疗,对照组只接受一般治疗,所有治疗组和对照组患者在入组时(治疗前)及在治疗观察12周后(治疗后)两次进行测定BODE指数,并对所获结果进行统计学分析.结果 治疗组患者治疗后较治疗前的BODE指数明显降低(P<0.05)、呼吸困难评分显著改善(P<0.05)、6分钟步行距离有极显著的提高(P<0.01),BMI和FEV1有所改善,但差异无统计学意义(P>0.05).对照组观察前后上述指标均无改善.结论 长期无创正压通气可有效降低重度COPD患者的BODE指数,改善患者的临床症状及预后.

Objective To evaluate the application value of long term non-invasive positive pressure ventilation in stable COPD patients.Methods Collect cases of 54 patients who have been diagnosed as COPD in stable period and are in accordance with the requirements of our study,and they were randomly divided into treatment and control groups,27 patients in each group.The treatment group was given general treatment and long term non-invasive positive pressure ventilation,and the control group was given simple general treatment.The BODE index was measured respectively before and after 12 weeks treatment in all patients of both groups.The results of experimental and control groups before and after treatment were statistically analyzed.Results The BODE index and MMRC significantly decreased after treatment in treatment group (P<0.05),6MWD increased markedly in treatment group (P<0.01).There was an improvement in BMI and FEV1,but this was not statistically significant (P>0.05).No marked change of all above-mentioned indexes (BODE index,MMRC,6MWD,BMI and FEV1) was observed in control groups.Conclusions.Long term non-invasive positive pressure ventilation can effectively reduce the BODE index of sever COPD patients and improve their clinical symptom and prognosis.
