目的 探讨汉族人群中难治性强迫症与非难治性强迫症的5-羟色胺转运体启动子区(5-HT-TLPR)基因多态性差异.方法 采用聚合酶链反应(PCR)和限制性片断长度多态性(RFLP)技术,检测51例难治性强迫症和59例非难治性强迫症的5-HTT基因型,并比较2组之间的5-HTT基因多态性差异.结果 难治性强迫症和非难治性强迫症组5-HTT的基因型(X2=11.09,P=0.02)和等位基因频率(X2=20.04,P=0.00)差异有显著性.L+/L+基因型和L+等位基因与难治性强迫症呈明显正关联(OR=9.39,X2=9.51,P<0.01;OR=23.40,X2=19.03,P<0.01.结论 5-HTT基因多态性中L+/L+基因型和L+与难治性强迫症相关联,是难治性强迫症的危险因子.

Objective To explore the difference of 5-HTTLPR polymorphism between refractory and non-refractory obsessive-compulsive disorder(OCD)in the Han nationality.Methods 51 refractory OCD patients and 59 non-refractory OCD patients were took part in this study.The gene polymorphism of 5-HTT in all subjects was genotyped directly via polymerase chain reaction(PCR)and restriction fragment length polymorphism(RFLP)technique,and to compare the differences of 5-HTT polymorphism between refractory and non-refractory OCD.Results There was significant difference between refractory and non-refractory OCD patients in the frequencies of genotype(X2=11.09,P=0.02)and allele(X2=20.04,P=0.00)for 5-HTTLPR;L+/L+ genotype and L+ Al- lele of 5-HTT was positively associated with OCD(OR=9.39,X2=9.51,P<0.01;OR=23.40,X2=19.03,P<0.01).Conclusion L+/L+ genotype and L+ allele of 5-HTT gene may be associated with OCD in the Han nationality,may be risk factor for refractory OCD.
