目的 观察海水浸泡对兔角膜爆炸穿孔伤后角膜上皮细胞超微结构的影响.方法 健康灰兔12只,爆竹致兔角膜爆炸伤并于角膜中周部全层切开,切口长约3mm.右眼为实验眼,对侧眼为对照眼.实验眼将海水通过角膜切口注入前房,海水持续灌注角结膜面30 min.对照眼使用生理盐水注入前房和灌注眼表.使用光镜及电镜分别观察造模后1d、3d、5d、7d、10 d、14 d两组角膜上皮细胞和基质层超微结构的变化.结果 实验组造模后1d至14 d,角膜上皮细胞胞浆出现不同程度的空泡样变性,微绒毛状突起消失或部分消失,染色质增多,线粒体肿胀,细胞核固缩、碎裂、崩解,基质层胶原纤维排列紊乱,胞核空泡样变性,其中3d改变最明显,10 d、14 d可见部分上皮细胞增生;相同时间段对照组改变明显轻于实验组,对照组7d、10 d大部分上皮细胞增生,14 d无明显异常改变.结论 角膜爆炸穿孔伤合并海水浸泡后角膜上皮细胞的超微结构改变明显重于对照组,提示海水浸泡加重角膜上皮细胞的损伤.

목적 관찰해수침포대토각막폭작천공상후각막상피세포초미결구적영향.방법 건강회토12지,폭죽치토각막폭작상병우각막중주부전층절개,절구장약3mm.우안위실험안,대측안위대조안.실험안장해수통과각막절구주입전방,해수지속관주각결막면30 min.대조안사용생리염수주입전방화관주안표.사용광경급전경분별관찰조모후1d、3d、5d、7d、10 d、14 d량조각막상피세포화기질층초미결구적변화.결과 실험조조모후1d지14 d,각막상피세포포장출현불동정도적공포양변성,미융모상돌기소실혹부분소실,염색질증다,선립체종창,세포핵고축、쇄렬、붕해,기질층효원섬유배렬문란,포핵공포양변성,기중3d개변최명현,10 d、14 d가견부분상피세포증생;상동시간단대조조개변명현경우실험조,대조조7d、10 d대부분상피세포증생,14 d무명현이상개변.결론 각막폭작천공상합병해수침포후각막상피세포적초미결구개변명현중우대조조,제시해수침포가중각막상피세포적손상.
Objective To observe the effect of seawater immersion on ultrastructural changes of the rabbit corneal epithelium after corneal explosive penetrating injury.Methods The animal model of corneal explosive penetrating injury was established in 12 health grey rabbits.For each rabbit,the corneas were incised in central with 3 rm incision,the right eye was taken as an experimental one with seawater immersion for 30 minutes and the left eye was taken as a control one with 0.9% sodium chloride immersion for the same time.The ultrastructural changes of corneal epithelium and stroma were observed on 1,3,5,7,10 and 14 day after modeling with light and electron microscope.Results Transmission electron microscope showed that vacuoles in the cytoplasm of corneal epithelium,losing of the microvillus,swelling of the mitochondria,pyknosis of the cellular nuclei and arranging in disorder of the stroma collagen.These ultrastructural changes of rabbit cornea was slighter in the control eye than the experiment eye at the same time after the rabbit eyes model was established.Conclusion The ultrastructural changes of the rabbit corneal epithelium was more marked in the experiment than the control.The seawater immersion can aggravate the injuries of the corneal epithelium.
