目的 在大鼠慢性背根神经节压迫模型中观察鞘内注射地塞米松的镇痛效果.方法 制作慢性背根神经节压迫模型(CCD组),同时制作不给予背根神经节压迫的假手术模(sham组).所有的实验大鼠在术前3d,术后第3、4、7、10、13、14、15天检测痛行为学指标.包括机械痛缩足阈值(paw withdrawal mechanical threshold,PWMT)和热痛缩足潜伏期(paw with-drawal themud latencv,PWTL).CCD模型组分为10%DWSO对照组(C组)、早期治疗组(ET组)、后期治疗组(LT组)和二次治疗组(TT组).10%DWSO对照组在术后第3天和第13大鞘内给予10%DWSO,早期治疗组于术后第3天分别给予地塞米松,第13天给予10%DWSO.后期治疗组于术后第3天给予10%DWSO,第13天给予地塞米松,二次治疗组术后第3、13天均给予地塞米松.结果 术后3 d空白对照组PWMT(2.67±1.03)较sham组PWMT(11.33±2.94)和术前基础值PWMT(13.33+2.58,13.33±2.58.13.33±2.58)有显著降低(P<0.05)、C组PWTL(8.82±1.82)较sham组(22.14±2.34)和基础值(22.52±1.94,21.59~2.49.22.46±1.52)明显缩短(P<0.05).鞘内给药之后ET组和TT组PWMT增高(P<0.05),PWTL延长(P<0.05);但LT组PWTL、PWMT无明显变化(P>O.05).结论 鞘内注射地塞米松可以有效改善慢性背根神经节压迫引起的痛行为学反应,但具有时间依赖性.

목적 재대서만성배근신경절압박모형중관찰초내주사지새미송적진통효과.방법 제작만성배근신경절압박모형(CCD조),동시제작불급여배근신경절압박적가수술모(sham조).소유적실험대서재술전3d,술후제3、4、7、10、13、14、15천검측통행위학지표.포괄궤계통축족역치(paw withdrawal mechanical threshold,PWMT)화열통축족잠복기(paw with-drawal themud latencv,PWTL).CCD모형조분위10%DWSO대조조(C조)、조기치료조(ET조)、후기치료조(LT조)화이차치료조(TT조).10%DWSO대조조재술후제3천화제13대초내급여10%DWSO,조기치료조우술후제3천분별급여지새미송,제13천급여10%DWSO.후기치료조우술후제3천급여10%DWSO,제13천급여지새미송,이차치료조술후제3、13천균급여지새미송.결과 술후3 d공백대조조PWMT(2.67±1.03)교sham조PWMT(11.33±2.94)화술전기출치PWMT(13.33+2.58,13.33±2.58.13.33±2.58)유현저강저(P<0.05)、C조PWTL(8.82±1.82)교sham조(22.14±2.34)화기출치(22.52±1.94,21.59~2.49.22.46±1.52)명현축단(P<0.05).초내급약지후ET조화TT조PWMT증고(P<0.05),PWTL연장(P<0.05);단LT조PWTL、PWMT무명현변화(P>O.05).결론 초내주사지새미송가이유효개선만성배근신경절압박인기적통행위학반응,단구유시간의뢰성.
Objective To investigate the analgesic effects of intrathecal dexamethasone injection on pain induced by chronic comDression of dorsal root ganglion in mouse.Methods The test model(CCD group)and sham model(sham group)of chronic dorsal root ganglion compression were established.Pain ethology indexes such as paw withdrawal mechanical threshold (PWMT)and Daw withdrawal thermal latency(PWTL)were assessed on 3 d before and 3 d,4d,7d.10 d,13 d,14d and 15 d after operation.The mice in group CCD were subdivided randomly into solventia group(C group),early treatment group(ET grouP),later treatment group(LT group)and twice treatment group(TT group).Vehicle(10%DMSO) and dexamethasone were intrathecally iniected at both 3 and 13 postoperative day in groups sham.C and group TT,respectively.Dexamethasone and vehicle were injected at 3 and 13 postoperative day,respectively in group ET.Vehicle and dexamethasone were injected at 3 and 13 postoperative day,respectively in group LT.Results Compared to group sham and preoperative level,PWMT and PWTL decreased at 3 postoperative day in group C(P<0.05).Intrathecal dexamethasone injection significantly increased PWMT and PWTL(P<O.05)in both groups ET and TT,however with no obvious change in pain ethology indexes in group LT.Conclusion Intrathecal dexamethasone injection alleviates pain behavior induced by chronic dorsal root ganglion compression in mouse in a time dependence manner.
