目的研究联合应用血液透析+血液灌流对维持性血液透析患者尿毒症脑病发生的预防效果.方法对45例符合入选条件的维持性血液透析患者按随机数字表法分成对照组(21例)和治疗组(24例).对照组行常规血液透析,治疗组使用常规血液透析加每月1次血液灌流进行串联治疗,每次血液灌流2～3 h,透析4.5 h,血液流量为200～250 ml/min,透析液流量500 ml/min,用肝素钠或低分子肝素钙抗凝,观察尿毒症脑病的发生情况.结果对照组有7例患者发生尿毒症脑病,而治疗组仅有1例患者发生尿毒症脑病,且生活质量明显改善.结论应用血液透析联合血液灌流可有效预防维持性血液透析患者尿毒症脑病的发生.

Objective To evaluate the efficacy of combined hemodialysis-hemoperfusion therapy compared with conventional dialysis in preventing uremic encephalopathy. Methods 45 enrolled patients on maintenance hemodialysis were randomized to receive conventional hemadialysis(control group,21 cases)or combined with monthly sequential hemoperfusion (study group,24 cases). The duration of hemoperfusion was 2-3 hours and 4.5 hours for use of hemodialysis. Blood flow rates were maintained at 200-250 ml/min and dialysate flow rate was at 500 ml/min. Anticoagulation was performed with heparin sodium or low-molecular-weight heparin calcium. Observation was made on incidence of uremic encephalopathy. Results 7 cases developed uremic encephalopathy in control group compared to only 1 case in study group, in which significant improvement of quality of life was also observed. Conclusion Use of hemodialysis-hemoperfusion therapy could effectively prevent the development of uremic encephalopathy in maintenance dialysis patients.
