目的 观察右美托咪啶应用于幕上病灶切除术后镇静作用的临床效果.方法 选择幕上病灶切除术后患者40例(均在麻醉恢复室),年龄18 ~60岁,随机分为两组,每组20例.试验组:10 min静脉泵注右美托咪啶0.8 μg/kg后,以0.6 μg·kg-1·h-1维持40 min;对照组:10 min静脉泵注生理盐水10 ml后,20ml生理盐水静脉泵注40 min.记录患者血压、心率、呼吸等变化,观察给药后患者镇静作用以及不良反应的发生情况.结果 试验组给入右美托咪啶后HR、MAP均低于基础值,对比对照组差异具有统计学意义(P<0.05);RR各个时间点则无明显变化;试验组均达到Ramsay镇静分级Ⅳ级状态.结论 右美托咪啶(负荷量0.8μg/kg,维持0.6μg·kg-1·h-1)应用于幕上病灶切除术后患者取得了满意的镇静效果.

목적 관찰우미탁미정응용우막상병조절제술후진정작용적림상효과.방법 선택막상병조절제술후환자40례(균재마취회복실),년령18 ~60세,수궤분위량조,매조20례.시험조:10 min정맥빙주우미탁미정0.8 μg/kg후,이0.6 μg·kg-1·h-1유지40 min;대조조:10 min정맥빙주생리염수10 ml후,20ml생리염수정맥빙주40 min.기록환자혈압、심솔、호흡등변화,관찰급약후환자진정작용이급불량반응적발생정황.결과 시험조급입우미탁미정후HR、MAP균저우기출치,대비대조조차이구유통계학의의(P<0.05);RR각개시간점칙무명현변화;시험조균체도Ramsay진정분급Ⅳ급상태.결론 우미탁미정(부하량0.8μg/kg,유지0.6μg·kg-1·h-1)응용우막상병조절제술후환자취득료만의적진정효과.
Objective To investigate the clinical sedation efficacy of dexmedetomidine for the patients undergoing supratentorial mass lesion postoperation.Methods 40 cases in the post-anesthesia care unit.ASA physical status I and Ⅱ.Patients underwent elective supratentorial mass lesion surgery who were randomly divided into two groups.Group A:Dexmedetomidine (an initial dose of 0.8 μg/kg for 10 min,followed by a continuous infusion of 0.6 μg · kg-1 · h-1 ).Group B:placebo saline was administered.MAP,HR,SpO2,RR,Ramesay assessment of sedation and adverse reaction were recored.Results The hemodynamic status of Group A was more influenced.Adverse reaction was significantly lesser in Group A.The experimental group at different time points on blood pressure,heart rate were lower than the control group,obvious difference in the two groups( P <0.05).Group A had small influence of MAP and RR.The degree of sedation which 0.6 μg · kg -1 · h -1 produced was quite suitable the clinical practice.Conclusions Dexmedetomidine ( an initial dose of 0.8 μg/kg for 10 min,followed by a continuous infusion of 0.6 μg · kg-1 · h-1 )has good sedative for the patients undergoing supratentorial mass lesion postoperation.
