目的 研究去甲肾上腺素预处理诱导内源性热休克蛋白70( HSP70)表达规律,探讨内源性HSP70对离体供心的保护作用,以寻求更为有效的延长供心保存的方法.方法 将60只SD大鼠按随机数字法分为实验组(腹腔注射去甲肾上腺素)和对照组(腹腔注射生理盐水),均对SD大鼠行腹腔注射,每24小时给药一次,共给药6次.2组均于最后1次给药24h后采用免疫组织化学法分别于供心离体后5个时间点(0、4、6、8、10 h)观察左心室前壁心肌组织的HSP70表达情况以及超氧化物歧化酶(SOD)和丙二醛(MDA)的含量.结果 随着时间的延长,2组HSP70表达逐渐下降,与对照组比较,实验组同时间点的HSP70表达量明显增高,组间差异有统计学意义(P<0.01).实验组的SOD活性明显高于各时间点相对应的对照组;但随着供心保存时间的延长,活性逐渐下降;实验组保存10 h后明显下降,而对照组保存6h下降明显.各时间点实验组心肌组织中MDA的含量明显低于对照组,组间差异有统计学意义[(0.35±0.09)μmol/g比(1.98±0.55) μmol/g,(0.35±0.09)μmol/g比(2.13±0.57) μmol/g,(0.52±0.10) μmol/g比(2.94±0.55)μmol/g,(1.28±0.24) μmol/g比(4.02 ±0.97) μmol/g,(2.70±0.41) μmol/g比(5.31±1.34)mol/g,P<0.01].实验组SOD比对照组同时间点的差异有统计学意义[(250.87±61.23) U/mg比(146.50±40.37) U/mg,(246.36 ±57.42) U/mg比(143.28±39.21) U/mg,( 235.85±54.33) U/mg比(118.46±35.81) U/mg,(205.61±43.26) U/mg比(79.37±21.65) U/mg,( 112.03±31.75) U/mg比(50.26±14.98) U/mg,P<0.01].结论 去甲肾上腺素预处理诱导心肌HSP70高表达对供心具有明显的保护效应,其MDA含量降低、SOD活性升高,增强心肌抗氧自由基损伤能力,从而发挥其对供心心肌细胞保护作用.

Objective To investigate protective effect of endogenous Heat Shock Protein(HSP) 70 in vitro for donor heart,and to find a more effective method to extend the donor heart preservation by pretreatment of Norepinephrine.Methods Sixty rats were divided into two groups randomly( thirty in each group).Experimental group:SD rats were injected Norepinephrine by introperitoneal once every 24 hours,which were administered 6 times.Control group: SD rats were injected normal saline by introperitoneal once every 24 hours,which were administered 6 times.SD rats in each group were killed for collecting myocardium tissue after twenty-four hours from the last injection.The expression of myocardial HSP70,superoxide dismutase(SOD) activity and malondialdehyde(MDA) contents were checked by immunohistochemistry.Results Compared with the control group,the expression of HSP70 was higher in experimental group at the five time points [ ( 9.36 ± 2.53 ) % vs ( 3.59 ± 1.01 ) %,( 9.09 ± 2.34) % vs (3.24±0.87)%,(8.65 ±2.11)% vs(2.08 ±0.56)%,(6.99 ± 1.77)% vs(1.06 ±0.26)%,(2.80 ±0.65 )% vs(0.25 ±0.05 )%,P <0.01 ].In the experimental group,the SOD activity in the myocardial tissue decreased significantly after preservation of ten hours,which equaled to the six-hours in the control group.Compared with the control group,the contents of MDA in the experimental group had a lower expression at the five time points,which showed a continuous increasing tendency following the preserved time.In the experimental group,the MDA contents in the myocardial tissue after being preserved ten hours had a significantly increase at the five time points [ (0.349 ± 0.092 ) nmol/mg vs ( 1.98 ± 0.55 ) nmol/mg,( 0.349 ± 0.092 ) nmol/mg vs ( 2.13 ± 0.57 ) nmol/mg,(0.525 ± 0.102 ) nmol/mg vs ( 2.94 ± 0.55 ) nmol/mg,( 1.283 ± 0.241 ) nmol/mg vs ( 4.02 ± 0.97 ) nmol/mg,(2.70 ± 0.415 ) nmol/mg vs ( 5.31 ± 1.34) nmol/ mg,P < 0.01 ].The activity of SOD in the experimental group had a higher expression at the five time points[ (250.87 ± 61.23) U/mg vs( 146.50 ± 40.37) U/mg,(246.36 ± 57.42) U/mg vs ( 143.28 ± 39.21 ) U/mg,(235.85 ± 54.33 ) U/mg vs ( 118.46 ± 35.81 ) U/mg,(205.61 ± 43.26 ) U/mgvs (79.37 ± 21.65 ) U/mg,( 112.03 ± 31.75 ) U/mg vs ( 50.26 ± 14.98 ) U/mg,P < 0.01 ],which showed a continuous decreasing tendency following the preserved time.Conclusions The HSP70 expression in myocardium is high with Norepinephrine preconditioning,which is a beneficial protection for donor heart by decreased contents of MDA,increased SOD activity and increased myocardial anti-oxidation.
