目的 探讨脑静脉畸形出血的影像学特点及其手术治疗方法.方法 回顾性分析8例脑静脉畸形出血的临床资料.7例采取显微手术治疗,1例出血破入脑室系统采取双侧脑室穿刺体外引流术.结果 8例经影像学及手术确诊为静脉畸形,其中2例合并海绵状血管瘤.随访2-60个月,平均21个月.所有患者均未发生再次出血,4例恢复日常生活工作,3例有不同程度恢复,但遗留神经功能缺损,1例加重(卧床).结论 静脉畸形MRI、DSA表现呈"水母头"改变.血肿量大、反复出血及神经功能障碍进行性加重是脑静脉畸形的手术指征,手术应尽量保留有正常引流功能的静脉畸形.

Objective To explore the imaging findings and surgical treatment of the cerebral venous malformations.Methods We retrospectively analyzed clinical datas of 8 cases of cerebral venous malformations presented with hemorrhage.Seven cases were taken microsurgical treatment;Another case with hemorrhage protrusion into cerebral ventricle were undertaken external ventricular drainage.Results Eight cases were confirmed as venous malformation by image examination and operation,two of which with cavernous angioma.During the follow-up period of 2-60 months,no hemorrhage occurred again.Four cases recover to normal state,3 patients' symptoms improved differently but left neurological defect,another one aggravated(lying in bed).Conclusion The characteristic of venous malformation in MRI and DSA is"caput medusae"sign.Large volume hemorrhage,repeated hemorrhage and neurological defect aggravating progressively aye seen as the sign of operation of cerebral venous malformation,the vein that has normal draining fuction should be retained during operation.
