目的探讨晚期胰腺癌的药物缺血方法。方法解剖犬胰腺、胰十二指肠前动脉,结扎近心端后,远心端插入化疗泵,术中、术后注射尿素注射液(200mg/kg体重,浓度为40%),分别于术后观察WBC计数、血淀粉酶、胰岛素、血糖和胰十二指肠的病理变化。结果术后早期WBC计数、血清淀粉酶升高(P<0.01),远期血糖及胰岛素波动范围不大,无统计学差别(P>0.05);胰腺病理变化表现为严重纤维化改变,十二指肠则表现为腺体轻度萎缩,扫描电镜粘膜有轻度萎缩。结论经胰十二指肠前动脉注射尿素可引起胰腺缺血,病理改变以纤维化为主,是一种安全可靠的手术方法。

목적탐토만기이선암적약물결혈방법。방법해부견이선、이십이지장전동맥,결찰근심단후,원심단삽입화료빙,술중、술후주사뇨소주사액(200mg/kg체중,농도위40%),분별우술후관찰WBC계수、혈정분매、이도소、혈당화이십이지장적병리변화。결과술후조기WBC계수、혈청정분매승고(P<0.01),원기혈당급이도소파동범위불대,무통계학차별(P>0.05);이선병리변화표현위엄중섬유화개변,십이지장칙표현위선체경도위축,소묘전경점막유경도위축。결론경이십이지장전동맥주사뇨소가인기이선결혈,병리개변이섬유화위주,시일충안전가고적수술방법。
Objective To explore the way of achieving ischemia by injecting urea solution in pancreatic carcinoma at the advanced stage. Methods The dog's pancrease and anterior artery of pacreaticoduodenum were dissected. The approximal artery was ligated and a chemotherapy pump was inserted into the distal artery. Both during and after the operation, urea solution was injected (20mg/
kg, 40 % in concentration). After the operation, special attention was paid to the WBC count, serum amylase, serum insulin, blood glucose and the pathological changes of pancrease and duodenum. Results Shortly after the operation,WBC count and serum amylase were increased (P < 0.01 ). Blood glucose and serum insulin showed no great changes, and there only atrophied slightly. Conclusions Injection of urea solution through the anterior artery of pancreaticoduodenum proved to be an effective and safe way to cause pancreatic ischemia, in which fibrosis was the main pathological change.
