目的 对比观察腹腔镜精索血管集束状结扎术与腹腔镜单纯精索内静脉结扎术治疗精索静脉曲张的疗效.方法 将70例精索静脉曲张患者随机分成两组,A组36例,行腹腔镜下精索血管集束状结扎术;B组34例,行单纯精索内静脉结扎术,对两组患者术前术后的临床表现、精子质量、手术时间进行对比分析.结果 两组病例治愈率相比差异有统计学意义(P<0.05);手术时间相比差异有统计学意义(P<0.01);精子数量、精子存活率明显提高(P<0.01),精子畸形率明显下降(P<0.01),但两组改善程度相比差异无统计学意义(P>0.05).结论 腹腔镜精索血管集束状结扎术与腹腔镜单纯精索内静脉结扎术治疗精索静脉曲张疗效可靠,但前者更具有操作简便、手术时间短、治愈率高等优点.

목적 대비관찰복강경정색혈관집속상결찰술여복강경단순정색내정맥결찰술치료정색정맥곡장적료효.방법 장70례정색정맥곡장환자수궤분성량조,A조36례,행복강경하정색혈관집속상결찰술;B조34례,행단순정색내정맥결찰술,대량조환자술전술후적림상표현、정자질량、수술시간진행대비분석.결과 량조병례치유솔상비차이유통계학의의(P<0.05);수술시간상비차이유통계학의의(P<0.01);정자수량、정자존활솔명현제고(P<0.01),정자기형솔명현하강(P<0.01),단량조개선정도상비차이무통계학의의(P>0.05).결론 복강경정색혈관집속상결찰술여복강경단순정색내정맥결찰술치료정색정맥곡장료효가고,단전자경구유조작간편、수술시간단、치유솔고등우점.
Objective To compare the efficicence between laparoscopic cluster ligation and high ligation of internal spermatic veins in treatment of varicocele.Methods Seventy examples were randomly divided into two groups, 36 patients (group A)who underwent laparoscopic cluster ligation and 34 patients(group B) who received laparoscopic high ligation of internal spermafic vein,and to analyse the sperm quality, surgery time and the clinical performance.Results Significant differences were found in the operation time(P<0.01)and healing rate between the two groups(P<0.05). Sperm number and sperm living rate were distinctly enhanced(P<0.01),and the rate of sperm deformity was significantly failed(P<0.01),but improvements were not significant different in the two grops(P>0.05).Conclusion Laparoscopic cluster ligation and high ligation of internal spermatic vein for the treatment of varicocele is reliable effect,but the former has the advantages of shorter operation time,simple orperation and higher cure rate when compared to the laster.
