目的:探讨介入下经椎旁阿霉素背根神经节注射治疗晚期癌痛的可行性.方法:42例椎体转移瘤患者在X线介入下经椎旁背根神经节注射0.33%阿霉素1～1.5 mL/节,观察术后1天、1周、1月、3月、6月视觉模拟评分(VAS)及疗效评估,术前、术后1周吗啡用量及相关并发症.结果:术后1天、3月VAS评分较术前有显著降低(P<0.05)、术后1周、1月较术前有非常显著降低(P<0.01);术后1天、1周、1月、3月、6月疗效优良率分别为59.5%、83.3%、76.2%、58.5%、33.3%;吗啡用量术后较术前有非常显著降低(P<0.01).结论:经椎旁背根神经节注射阿霉素治疗晚期癌痛效果确切,操作简单、并发症少,中、远期效果不稳定.
목적:탐토개입하경추방아매소배근신경절주사치료만기암통적가행성.방법:42례추체전이류환자재X선개입하경추방배근신경절주사0.33%아매소1～1.5 mL/절,관찰술후1천、1주、1월、3월、6월시각모의평분(VAS)급료효평고,술전、술후1주마배용량급상관병발증.결과:술후1천、3월VAS평분교술전유현저강저(P<0.05)、술후1주、1월교술전유비상현저강저(P<0.01);술후1천、1주、1월、3월、6월료효우량솔분별위59.5%、83.3%、76.2%、58.5%、33.3%;마배용량술후교술전유비상현저강저(P<0.01).결론:경추방배근신경절주사아매소치료만기암통효과학절,조작간단、병발증소,중、원기효과불은정.
Objective:To explore the feasibility of X - ray guiding paravertebral injection of adriamycin into dorsal root ganglion in treating advanced stage of cancer pain.Methods:0.33% adriamycin(1～1.5 mL/knot) was injected in paravertebral dorsal root ganglion of 42 patients with vertebral metastasis.All procedures were guided by X-ray.The effect and VAS of 1 day、1week、1 month、3 month、6 months were evaluated and the relative complications and the dosage of morphine of 1 week before and after treatment were observed.Result:The VAS of 1 day、3 month were significantly lower than before treatment (P <0.05) and the VAS of 1 wsek、1 month were greatly significantly lower than before treatment (P<0.01).The effective rate of treatment were 59.5%,83.3%,76.2%,58.5%,33.3%,respectively in 1 day、1 week、1 month、3 month、6 months after treatment.The dosage of morphine was greatly significantly lower than before treatment (P <0.01).Conclusion:The clinical effect of paravertebral injection of adriamycin into dorsal root ganglion in treating advanced stage of cancer pain was accurate.This method was operated simply with less complications,but the medium and long - term effects was unstable.
