目的 探讨激光治疗对糖尿病视网膜病变(DR)患眼血流动力学的影响.方法 采用Zeiss Visulas 532s眼科激光治疗仪对35例(56眼)DR行泛视网膜光凝治疗,用彩色多普勒超声检测激光治疗前后DR患眼眼动脉(OA)和视网膜中央动脉(CRA)的血流状态.并与30例(60眼)正常人的CA和CRA血流进行对照.结果 DR组与对照组相比,CA和CRA的收缩期血流峰速,舒张末期流速及平均流速均降低,阻力指数增高;激光治疗后DR患眼CA和CRA收缩期血流峰速,舒张末期流速,平均流速均降低,阻力指数增高.结论 糖尿病视网膜病变患者CA和CRA的血流速度降低,视网膜循环阻力升高,视网膜血液供应不良.激光治疗导致DR患眼视网膜血流降低.

Objective To study the effect of laser photocoagulation on the retinal hemedynamics in patients with diabetic retinopathy(DR).Methods The hemodynamic parameters of ophthalmic artery (CA)and central retinal artery(CRA)were measured in the diabete mellitus group with color Doppler flow imaging(CDFI)before and after laser photocoagulation.And the hemodynamic parameters of CA and CRA in 30 normal cases(60 eyes)Was measured as control.Results The peak systolic velocity(PSV),end diastolic velocity(EDV) and mean velocity(Vm) were decreased in the ophthalmic artery and the central retinal artery ofthe patients with DR,but the resistance index (RI) were increased.PSV,EDV and Vm in the ophthalmic artery and the central retinal artery were decreased after panretinal photocoagulation,but RI were increased.Conclusions The blood flow velocities were decreased in the ophthalmic artery and in the central retinal artery of the patients with DR.The retinal circulation resistance increased and ischemia occurred in retina.photocoagulation resulted in a reduction in ocular blood flow velocities.
