目的 探讨二维高频超声及彩色多普勒超声(CDFI)钼靶X线联合应用对乳腺癌诊断的适用价值.方法 回顾性分析及病理证实的乳腺肿块76例,其中乳腺癌26例,乳腺良性肿瘤50例.对其二维超声图像特征、CDFI、钼靶X线特征进行分析.结果 乳腺癌与良性肿瘤二维超声图像方面差异有显著性.乳腺癌二维呈低回声,非均质肿物;边界不清,边缘毛刺,强回声晕,边缘成角,后方衰成,有声影,内部微小钙化,导管内结构紊乱,内呈小分叶,边缘增厚.钼靶X线检查发现肿块、微小钙化、局部结构紊乱是乳腺癌的X线直接征象,局灶性致密浸润,局部皮肤增厚,导管增粗影及腋窝淋巴结增大是乳腺癌的X线间接征象,二维超声结合CDFI比钼靶x线检查准确.CDFI:血流没有显著性.联合运用较单独用一种的敏感度、特异度和准确度均有提高.结论 二维超声结合CDFI比钼靶X线检查更准确.联合应用可以提高乳腺癌诊断的准确率,是乳腺影像学最佳组合.

Objective To investigate the value of nigh frequency 2-Dimension combined with color doppler flow imaging(CD-FI) mammograpby on the diagnosis of breast cancer. Methods Seventy-six cases were studied retrospectively including 26 malig-nant cases and 50 benign cases. The diagnosis by using high frequency probe combined with CDFI compared with that by using mam-mog-raphy respectively. Results There were significart difference between malignant and benign breast lesions in the appearance of the lesions. The masses, micro-calcification and structural disorder in duct were shown shown on the imaging of breast cancer.The miro-calcification and focai structural disorder were iound to be the direct mammographic signs of breast cancer in mammogra-phy. The indirected mammographic signs included focal compact infiltration, regional thicken skin widen ductal shadow and enlarged axillang lymph nodes. CDFI:There was no significant difference in the extent of blood flow between malignancy and benignancy. The combination of two techniques had higher sensitivity, specificity, and accuracy. Conclusion 2-Dimension and CDFI combined with mammography can raise diagnostic rate of breast cancer Their combination is the best one of berast imaging examinations.
