目的：探讨降钙素原（PCT）在行人工关节置换术的类风湿性关节炎患者术后早期感染诊断中的临床意义。方法检测41例类风湿性关节炎行人工关节置换术患者术前、术后48 h时血清PCT、CRP和ESR的水平，同时将术中关节液、术后48 h时切口渗液、引流管引流液分别进行细菌培养。根据术后48 h时切口渗液的培养结果，将患者分为感染组和未感染组，对两组患者的检测结果进行比较分析。结果术后ESR和CRP检测值在感染组和未感染组之间比较有所差异，但差异无统计学意义（P＞0．05）。术后感染组的PCT值明显高于未感染组，两组间的差异具有统计学意义（P＜0．001）。结论 PCT可以作为类风湿性关节炎人工关节置换术后早期感染的检测指标，对该类手术术后早期切口感染的诊断具有重要的临床价值。
목적：탐토강개소원（PCT）재행인공관절치환술적류풍습성관절염환자술후조기감염진단중적림상의의。방법검측41례류풍습성관절염행인공관절치환술환자술전、술후48 h시혈청PCT、CRP화ESR적수평，동시장술중관절액、술후48 h시절구삼액、인류관인류액분별진행세균배양。근거술후48 h시절구삼액적배양결과，장환자분위감염조화미감염조，대량조환자적검측결과진행비교분석。결과술후ESR화CRP검측치재감염조화미감염조지간비교유소차이，단차이무통계학의의（P＞0．05）。술후감염조적PCT치명현고우미감염조，량조간적차이구유통계학의의（P＜0．001）。결론 PCT가이작위류풍습성관절염인공관절치환술후조기감염적검측지표，대해류수술술후조기절구감염적진단구유중요적림상개치。
Objective To explore the application of procalcitonin in the diagnosis of early infection in patients with rheumatoid ar-thritis after arthroplasty.Methods Forty-one cases of patients with rheumatoid arthritis undergoing arthroplasty were selected.The level of se-rum PCT,CRP and ESR were detected before and after the operation,and the synovia in the operation,the drainage from drainage tube and the transudate from incision 48 h after operation were assembled and sent for bacterial culture,respectively.According to the culture result of the transudate from incision,patients were divided into infected group and uninfected group,and the test results were compared between the two group.Results The ESR and CRP values detected after the operation were different between infected and uninfected group,but the difference had no statistical significance (P>0.05).After the operation,the PCT value of infected group was higher than that of uninfected group,and the difference between the two groups had statistical significance (P<0.001).Conclusion PCT can be used as a better indicator for early infection after arthroplasty for rheumatoid arthritis,and it has important clinical value in the diagnosis of early infection after operation.
