目的研究大鼠经腹腔内注射胰腺炎相关性腹水(pancreatitis associated ascetic fluid,PAAF)诱导的急性肺损伤模型,旨在探讨该模型的意义.方法制备大鼠急性出血坏死性胰腺炎并收集PAAF.大鼠腹腔内注射PAAF诱导肺损伤,分别于注射后7 h、12 h活杀,观察肺组织结构以及胰腺病理学改变,并测定大鼠呼吸频率、动脉血气、外周血白细胞计数以及血清淀粉酶浓度.结果腹腔注射PAAF后,大鼠呼吸频率加快,PaO2水平显著下降,外周血白细胞计数升高;肺组织出现明显的损伤,表现为肺间质水肿以及大量的炎性细胞浸润;而胰腺病理学上改变轻微.结论建立了PAAF诱导的大鼠肺损伤模型.PAAF腹腔注射后引起的胰腺病变程度轻微,表明该模型能够一定程度上"屏蔽"胰腺病变对肺脏的影响,从而为研究肺损伤机制提供了比较单纯的研究背景.

Objective To explore the animal model of acute lung injury induced by pancreatitis associated ascetic fluid in SD rats and its significance.Methods Rats were induced with acute hemorrhagic necrotic pancreatitis(AHNP)to create PAAF.The animal model of acute lung injury in rats was established by injection of PAAF into their abdominal cavity.Respiratory frequency,Artery blood gas analysis,peripheral blood white blood cell count,serum amylase,and the change of structure of pancreas and lungs were observed.Results After injection,the respiratory frequency and peripheral white blood cell count in rats was higher,and the level of PaO2 decreased obviously and that of the PaCO2 increased significantly.The typical pathological changes of lung injury were found.Morphological examination demonstrated that a great deal of inflammatory cell infiltrated in the alveoli and interstitial tissue,and interstitial edema occurred in the lungs.The changes of structure of pancreas were not obvious,only tissue edema and inflammatory cell occurred.Conclusion The animal model of acute lung injury induced by PAAF was established successfully.The pathological changes of pancreas of rats induced by PAAF are slight,which suggests that this animal model can mask the affect of pancreatic pathological changes to lung,so as to provide a simple studying background for the study of pathogenesis of acute lung injury.
