目的探讨急性淋巴细胞白血病(ALL)和慢性髓性白血病(CML)易感性与HLA-DRB1基因多态性之间的关联性,找出急性淋巴细胞白血病和慢性髓性白血病的易感基因。方法采用序列特异性引物聚合酶链反应(PCR-SSP)DNA分型技术对56例ALL患者、48例CML患者和105例健康对照进行了HLA-DRB1基因分型。结果 ALL患者组与HLA-DR7基因关联,基因频率为24.1%,RR =2.56,χ2=7.34,P<0.01;CML患者组与HLA-DR4基因关联,基因频率为22.9%,RR =5.076,χ2=17.88,P<0.01;其他等位基因频率在实验组与对照组间差异无显著性。结论提示在河南汉族人群中,HLA-DR7与ALL有关联,HLA-DR4与CML有关联。

Objective To explore the possible association between HLA-DRB1 allele polymorphism and susceptibility to acute lymphoblastic leukemia(ALL) as well as chronic myelogenous leukemia(CML) in Chinese He′nan Han People and to find the predisposing genes of ALL and CML. Methods 105 normal people and 56 ALL patients and 48 CML patients of He′nan Han nationality were tested for HLA-DRB1 by polymerase chain reaction with sequence specific primers(PCR-SSP). Results Compared with normal controls, the frequency of HLA-DR7 was significantly increased in ALL group with a relative risk (RR) as 2.56 (χ2 =7.34, P<0.01) and with an etiological fraction (EF) as 0.29; the frequencies of other DRB1 alleles were not significantly increased in the ALL group. The frequency of HLA-DR4 was significantly increased in CML patient group, with a relative risk (RR) as 5.076 (χ2=17.88, P<0.01) and with an etiological fraction (EF) as 0.18, while the frequencies of other DRB1 alleles were not significantly increased in CML group. Conclusion These results showed that susceptibility to ALL is positively related to HLA-DR7 gene and susceptibility to CML is strongly correlated with HLA-DR4 gene.
