目的:研究哺乳期乳腺生理解剖与声像图的关系,及超声普查哺乳期乳腺的价值。方法:用高频彩超检查分娩4天到一年内,哺乳期乳腺60例120只乳房;离体乳汁声像图模拟试验。结果:哺乳期激乳素使乳腺小叶增生肥大,乳汁不断分泌、贮存;声像图见乳房增大,脂肪变薄;乳腺呈相对低回声与较密欠均匀、中小粗点状、结节状的高回声交融,成弥漫的雾状。乳汁充盈输乳管回声多样。乳头的多种组织结构与弹性肌束,形成密集,细小,点状,中高与相对低回声。乳汁充足血管丰富,流速快,RI低;乳汁不足血管细、少,流速低,RI高。声学密度定量乳汁测值高于水,低于乳腺间质,乳头增高,纤维斑块更高;反映了组织结构的不同。乳管内乳汁中脂肪滴缓缓流动为乳汁动力学表现。哺乳期乳腺结构异常回声28.3%(34/120只),输乳管排出受阻可致乳腺炎、输乳管扩张,乳汁潴留等异常回声。结论:哺乳期乳腺超声表现与生理解剖及乳汁分泌关系密切;超声普查对指导母乳喂养、防治乳腺病变有很大价值。

목적:연구포유기유선생리해부여성상도적관계,급초성보사포유기유선적개치。방법:용고빈채초검사분면4천도일년내,포유기유선60례120지유방;리체유즙성상도모의시험。결과:포유기격유소사유선소협증생비대,유즙불단분비、저존;성상도견유방증대,지방변박;유선정상대저회성여교밀흠균균、중소조점상、결절상적고회성교융,성미만적무상。유즙충영수유관회성다양。유두적다충조직결구여탄성기속,형성밀집,세소,점상,중고여상대저회성。유즙충족혈관봉부,류속쾌,RI저;유즙불족혈관세、소,류속저,RI고。성학밀도정량유즙측치고우수,저우유선간질,유두증고,섬유반괴경고;반영료조직결구적불동。유관내유즙중지방적완완류동위유즙동역학표현。포유기유선결구이상회성28.3%(34/120지),수유관배출수조가치유선염、수유관확장,유즙저류등이상회성。결론:포유기유선초성표현여생리해부급유즙분비관계밀절;초성보사대지도모유위양、방치유선병변유흔대개치。
Objective:To study the relationship between physiological anatomyand ultrasonic images, and the value of ultrasonic mass survey of mammary gland in lactation.Methods:Sixty cases from 4 days to 1 year postpartum,which meant 120 breasts were checked by high frequency color Doppler ultrasound.The isolated experiment of milk was also performed.Results:In lactation,the lactogen made the lobules of mammary gland hypertrophy,hyperplasia and full of milk. The ultrasonic image showed the breast enlarged and the fat turned thinner. The milk was secreted and stored constantly. The mammary gland has an echo pattern of hypoechoic mixed with hyperehoic fine, moderate and thick dot nodules so it appeared like a diffused mist. The lactiferous ducts was dilated by milk and showed various echo patterns. The various structures of the nipple and elastic muscle were concentrated to small hyperechoic dots and hypoechoic pattern. It was full of blood,with high speed and low RI when the milk was abundant. When it was short of milk, the blood vessels were rare with low blood speed and high RI. The acoustic density of milk was higher then that of water but lower than that of mesenchyma. The density of the nipple was high and the fibrous plaque was even more. So the difference of tissue structure was thus reflected. The slow flow of fat drop in milk was the appearance of its dynamics. The abnormality of mammary gland structure in lactation was high as 28%(34/120). The block of lactiferous ducts might result in mastitis,dilation of lactiferous ducts,milk stagnation and other abnormal echoes.Conclusions: By ultrasonic mass survey,the relationship between ultrasonic structure of mammary gland in lactation and milk secretion is clear.Thus,the ultrasonic mass survey was highly valuable in guiding breast feeding and prevention of mammary gland diseases.
