目的:探讨平流式轴流血泵在体实验中的泵性能及对心衰动物的辅助功能。方法:采用选择性结扎冠脉的方法对20只雄性健康成年绵羊建立心衰模型后,随机分为2组,运用一平流式轴流血泵对实验组动物进行左心辅助,于结扎冠脉前、心衰即刻和心衰后1 h测量两组动物各项血液动力学参数,评价平流血泵的动力学输出和对血流动力学参数的影响,探讨其对衰竭心脏的辅助机制,动物处死后取标本进行光、电镜检查,检测主要器官和血泵的栓塞情况。结果:1.可以采用结扎冠脉的方法建立左心辅助心衰动物模型;2.血泵的压力、流量输出能够达到辅助要求,血泵辅助流量占总流量的66%;3.并联于左心房和降主动脉的血泵可以将主动脉舒张压提高70%;4.平流式血泵主要通过分担心脏对外做功减少心肌耗氧,使心肌恢复;病理检查结果与血流动力学结果推测一致。5.尸检肾脏有散在白色斑块;血泵的进、出口与管道接口处覆盖一层薄薄白膜,整个过程血泵表面温度变化较小。结论:自制平流血泵可以改善心脏功能并可对心衰动物进行短期辅助。

Objective:In order to test the mechanical and assistant performance of the non-pulsatile pump,20 adult health sheeps were chosen in this vivo experiment.Method:After acute left heart failure was induced by selective ligation of the coronary artery,20 sheeps were divided into two groups:control and assistance groups.The sheeps in the assistance group were assisted by a non-pulsatile blood pump.During the operation,several blood dynamic parameters were measured,after sheep died,the myocardium at left heart apex was cut and investigated by microscopy and electric microscopy.Results:1.The animal model of an acute left heart failure for LVAD can be achieved by selective ligation of coronary arteries.2.The pump's output was able to assist a failure heart,it took up 66% of the total output.3.The mechanical assistance of the pump could increase the diastolic aortic pressure more than 70%.4.The non-pulsatile blood pump could decrease the oxygen consuming of the myocardium by sharing flow rate and improve the recovery of the failure heart.The results of the microscopy and electric microscopy were identical with the result of the blood dynamic parameters.5.After sheep died,a little thrombus were discovered at the kidney and inlet or outlet of the pump,the temperature on the pump surface changed in a little range during the operation.Conclusion:the non-pulsatile pump could reform the heart function and be used in assistant support.
