目的 通过观察2型糖尿病患者血白细胞介素-6(IL-6)、同型半胱氨酸(Hcy)的水平变化及其与尿白蛋白排泄率(UAER)的关系,探讨其在糖尿病肾病发病机制中的作用.方法 选取2型糖尿病患者90例(所有患者均符合2005年美国糖尿病协会诊断及分型标准),根据UAER分为单纯糖尿病组、早期肾病组和临床肾病组三组,并选取健康查体者30名作为对照组.所有受检者均测定UAER、IL-6、Hcy、空腹血糖、空腹胰岛素水平.结果 糖尿病组血IL-6、Hcy水平明显高于对照组(P<0.01),糖尿病肾病组血Hcy、IL-6水平显著高于单纯糖尿病组(P<0.01),IL-6、Hcy水平与胰岛素敏感性呈负相关、与UAER呈正相关.结论 随着UAER的增加,血IL-6、Hcy的水平逐渐升高,血IL-6、Hcy可能参与了糖尿病肾病的发病过程,参与糖尿病的微血管病变.

목적 통과관찰2형당뇨병환자혈백세포개소-6(IL-6)、동형반광안산(Hcy)적수평변화급기여뇨백단백배설솔(UAER)적관계,탐토기재당뇨병신병발병궤제중적작용.방법 선취2형당뇨병환자90례(소유환자균부합2005년미국당뇨병협회진단급분형표준),근거UAER분위단순당뇨병조、조기신병조화림상신병조삼조,병선취건강사체자30명작위대조조.소유수검자균측정UAER、IL-6、Hcy、공복혈당、공복이도소수평.결과 당뇨병조혈IL-6、Hcy수평명현고우대조조(P<0.01),당뇨병신병조혈Hcy、IL-6수평현저고우단순당뇨병조(P<0.01),IL-6、Hcy수평여이도소민감성정부상관、여UAER정정상관.결론 수착UAER적증가,혈IL-6、Hcy적수평축점승고,혈IL-6、Hcy가능삼여료당뇨병신병적발병과정,삼여당뇨병적미혈관병변.
Objective By observing the changes of serum interleukin-6 and homocysteine in the patients with type 2 diabetes and the relationship between the levels of interleukin-6,homocysteine and urinary albumin excretion rate (UAER),to study their roles in the pathogenesis of diabetic nephropathy.Methods According to the urinary albumin excretion rate,90 patients with type 2 diebetes were divided into three groups,simple diabetic group,early stage diabetic nephropathy and the clinical nephropathy group,30 healthy people were choosed as control group.The UAER,IL-6,Hcy,FBG,FINS were detected.Results The levels of IL-6,Hcy were higher in T2DM group than those in control group (P <0.01 ),and were higher in diabetic nephropathy group than those in simple diabetic group ( P < 0.01 ).The levels of IL-6,Hcy were negatively correlated with insulin sensitivity,and positive correlated with UARE.Conclusions IL-6,Hcy may be involved in the pathogenesis of diabetic nephropathy and diabetic microangiopathy.
