Idiopathic interstitial pneumonias Honeycomb lung%Ground-glass opacity
目的 通过对不同影像学特发性间质性肺炎(IIP)患者及正常人外周血几种细胞因子及肺功能对比研究,探讨细胞因子在IIP进展中的作用.方法 对蜂窝肺组、磨玻璃影组IIP患者及对照组外周血白介素1(IL-1)、白介索2受体(IL-2R)、IL-5、IL-6、干扰素(IFN)、肿瘤坏死因子(TNF)水平及用力肺活量(FVC)占预计值%、第1秒用力呼气容积(FEV1)占预计值%、FEV1/FVC、肺一氧化碳弥散量(DLCO)占预计值%进行测定.结果 ①蜂窝肺组患者外周血IL-1、IL-2R、IL-5、IL-6、TNF水平均较对照组升高(P<0.05或P<0.01);磨玻璃影组外周血IL-5、TNF水平均较对照组升高(P<0.05或P<0.01);蜂窝肺组患者外周血IL-2R、IL-5、TNF水平均较磨玻璃影组升高(P<0.05或P<0.01).②与对照组比较,蜂窝肺组和磨玻璃影组FVC占预计值0A、FEV1占预计值%、DLCO占预计值%均下降(P值均<0.01),磨玻璃影组FEV1/FVC较对照组升高(P<0.05),磨玻璃影组FVC占预计值%较蜂窝肺组降低(P<0.05).结论 外周血IL-1、IL-6、IL-2R水平在一定程度上可作为预测肺纤维化进展的标志物.

목적 통과대불동영상학특발성간질성폐염(IIP)환자급정상인외주혈궤충세포인자급폐공능대비연구,탐토세포인자재IIP진전중적작용.방법 대봉와폐조、마파리영조IIP환자급대조조외주혈백개소1(IL-1)、백개색2수체(IL-2R)、IL-5、IL-6、간우소(IFN)、종류배사인자(TNF)수평급용력폐활량(FVC)점예계치%、제1초용력호기용적(FEV1)점예계치%、FEV1/FVC、폐일양화탄미산량(DLCO)점예계치%진행측정.결과 ①봉와폐조환자외주혈IL-1、IL-2R、IL-5、IL-6、TNF수평균교대조조승고(P<0.05혹P<0.01);마파리영조외주혈IL-5、TNF수평균교대조조승고(P<0.05혹P<0.01);봉와폐조환자외주혈IL-2R、IL-5、TNF수평균교마파리영조승고(P<0.05혹P<0.01).②여대조조비교,봉와폐조화마파리영조FVC점예계치0A、FEV1점예계치%、DLCO점예계치%균하강(P치균<0.01),마파리영조FEV1/FVC교대조조승고(P<0.05),마파리영조FVC점예계치%교봉와폐조강저(P<0.05).결론 외주혈IL-1、IL-6、IL-2R수평재일정정도상가작위예측폐섬유화진전적표지물.
Objective To explore the roles of cytokines in the progress of idiopathic interstitial pneumonia (IIP) through detecting the concentration of several cytokines in peripheral blood and lung function of IIP patients with different imaging and normal persons. Methods The concentration of interleukin-1 (IL-1), interleukin-2 receptor (IL-2R), IL-5, IL-6, interferon (IFN), and tumor necrosis factor (TNF) in peripheral blood and lung function including forced vital capacity/predicted value (FVC pred%), forced expiratory volume in one second/predicted value (FEV^ pred%) , forced expiratory volume in one second/forced vital capacity ( FEV, /FVC), diffusing capacity of the lung for carbon monoxide/predicted value (DLCO pred%) were determined in honeycomb lung group, ground-glass opacity group,and normal control group. Results The concentration of IL-1,IL-2R,IL-5,IL-6, and TNF in peripheral blood of honeycomb lung group were higher than those in normal control group ( P <0. 05 or P <0. 01). The concentration of IL-5 and TNF in peripheral blood of ground-glass opacity group were higher than those in normal control group ( P <0. 05 or P <0. 01). The concentration of IL-2R,IL-5,and TNF in peripheral blood of honeycomb lung group were higher than those in ground-glass opacitygroup ( P <0. 05 or P <0. 01). Compared with normal control group, FVC pred% , FEV, pred%, and DLCO pred% were lower in honeycomb lung group and ground-glass opacity group (all P < 0. 01).FEV1/FVC in ground-glass opacity group was higher than that in normal control group ( P <0. 05).FVC pred% in honeycomb lung group was lower than that in ground-glass opacity group ( P <0. 05).Conclusions The concentration of IL-1, IL-6, and IL-2R in peripheral blood can be regarded as the makers to predict the progress of pulmonary fibrosis to some extent.
