目的应用超声检查探讨左侧精索静脉曲张(VC)引起男性不育患者睾丸动脉改变与精液质量之间的关系.方法采用能量多普勒频谱(PDUS)对符合入选标准的93例左侧精索静脉曲张患者及30例健康志愿者双侧睾丸包膜动脉(CA)的收缩期峰值流速(PSV)、阻力指数(RI)等参数进行测量,精液则做常规检查.VC按照临床触诊及超声检查结果分为Ⅰ～Ⅲ(VCI、VCⅡ、VCⅢ).结果与对照组比较,病例组左侧CA-PSV、RI均明显升高(P<0.01),而精子密度则显著降低(P<0.01).VC组内比较,随着VC级别的递增,左侧CA-PSV、RI逐渐增高,而精子密度则渐降.相关分析显示:VC组左侧CA-PSV、RI均分别与精子密度呈负相关(r=-0.665,P<0.05;r=-0.627,P<0.05).结论超声对左侧CA-PSV、CA-RI的评价可作为预测左侧VC不育患者精液质量的参考指标.
목적 응용초성검사탐토좌측정색정맥곡장(VC)인기남성불육환자고환동맥개변여정액질량지간적관계.방법 채용능량다보륵빈보(PDUS)대부합입선표준적93례좌측정색정맥곡장환자급30례건강지원자쌍측고환포막동맥(CA)적수축기봉치류속(PSV)、조력지수(RI)등삼수진행측량,정액칙주상규검사.VC안조림상촉진급초성검사결과분위Ⅰ～Ⅲ(VCI、VCⅡ、VCⅢ).결과 여대조조비교,병례조좌측CA-PSV、RI균명현승고(P<0.01),이정자밀도칙현저강저(P<0.01).VC조내비교,수착VC급별적체증,좌측CA-PSV、RI축점증고,이정자밀도칙점강.상관분석현시:VC조좌측CA-PSV、RI균분별여정자밀도정부상관(r=-0.665,P<0.05;r=-0.627,P<0.05).결론 초성대좌측CA-PSV、CA-RI적평개가작위예측좌측VC불육환자정액질량적삼고지표.
Objective To observe the relation between the related parameters of testicular artery changes detected by ultrasonography and the semen quality of male infertile patients caused by left varicocele(VC).Methods Power doppler ultrasonography spectrum(PDUS) was utilized to measure the parameters of the peak systolic velocity(PSV) and resistance index(RI) of bilateral testicular capsular artery(CA),and the routine examinations of semina were performed.VC was divided into three grades from grade I to grade Ⅲ (VCI,VCⅡ and VCⅢ) according to the manual palpation and ultrasonography.Results Compared with the control group,the left CA-PSV and CA-RI of VC group were significantly higher(P<0.01),and the sperm density was significantly lower(P<0.01).The left CA-PSV and CA-RI showed increasing tendency along the increase of VC severity,but the sperm density became decreasing.The linear correlation analysis showed that the left CA-PSV and CA-RI of VC group were respectively negatively correlated with the sperm density(r=-0.665,P<0.05;r=-0.627,P<0.05).Conclusions The left CA-PSV and CA-RI detected by ultrasonography may be as a reference index for forecasting the semen quality of left VC infertile patients.
