目的 评价血液灌流(hemoperfusion,HP)清除口服泰诺中毒患者血清中泰诺主要成分的效果.方法 采用股静脉插管急诊建立血液通路,利用血泵为动力、炭肾为吸附剂对2例急性泰诺中毒患者进行HP治疗,并动态观察血清中乙酰氨基酚等药物成分浓度的变化,测定血液灌流器(炭肾)中所吸附的泰诺主要成分的吸附量.结果 HP治疗能使患者体内的泰诺有效成分快速下降,病例1血清中对乙酰氨基酚浓度从HP开始前的13.4 μg/L降为HP结束时的5.81μg/L;病例2血清中对乙酰氨基酚浓度别从HP开始前的51.1 μg/L降为HP结束时的22.3 μg/L.病例1的炭肾对乙氨基酚的吸附量为119 542 μg,病例2为332 154 μg.结论 HP能在短时间内有效降低口服泰诺中毒患者体内的泰诺主要成分的含量,HP治疗急性泰诺中毒有效.

Objective To explore the effect of hemoperfusion (HP) on tylenol poisoned patients.Methods Urgently established the blood access by transfemoral catheterization of femoral vein,we used charcoal hemoperfusion by blood pump and dynamically monitored the plasma concentration of tylenol activingredients for the 2 patients and the content of tylenol activingredients in the charcoal was determined.Results Plasma concentration of tylenol activingredients of the 2 patients was declined gradually during and after the HP management.The acetaminophen serum concentration of the case 1 was declined from the13.4 μg/L at the start of HP to the 5.81 μg/L at the end of HP;and the case 2 was declined from 51.1 μg/L to 22.3 μg/L.The adsorption amount of acetaminophen in the blood perfusion device are respectively 119 542 μg of case 1 and 33 2154 μg of case 2.Conclusion Early hemoperfusion should be carried out for acute tylenol poisoning patients if there were indications,hemoperfusion can clear the tylenol activingredients and this is an effective measure to eliminate tylenol aetivingredients
