目的 探讨超声血管造影(USA)检测痉挛状态下大脑动脉内径、流速以及血流量的作用,为临床提供一种准确、无创的测量脑血管直径及血流量的方法.方法 18只兔颅骨开窗,建立蛛网膜下腔出血(SAH)模型,利用USA技术测量SAH建模前及建模后大脑中动脉(MCA)流速,用图像分析软件Getcolorpixel分析大脑中动脉(MCA)血管内径,并利用公式计算MCA的血流量,对照研究.结果 (1)在二维超声(2DUS)模式下USA测量MCA血管内径变化分别为SAH建模前(713±10.1)μm和SAH建模后3 d(475±6.4)μm,有明显差异(P<0.05).同时超声多普勒测得其流速分别为(34.67±3.11)cm/s和(55.4±3.81)cm/s(P<0.05).(2)计算血流量,分别为SAH建模前为(13.86±1.11)×10~(-2) ml/s,SAH建模后3 d为(9.19±0.71)×10~(-2) ml/s;二者差异显著(P<0.01).结论 USA可用于脑血流量测量,对SAH后血管痉挛及脑血流变化具有一定的诊断价值.

목적 탐토초성혈관조영(USA)검측경련상태하대뇌동맥내경、류속이급혈류량적작용,위림상제공일충준학、무창적측량뇌혈관직경급혈류량적방법.방법 18지토로골개창,건립주망막하강출혈(SAH)모형,이용USA기술측량SAH건모전급건모후대뇌중동맥(MCA)류속,용도상분석연건Getcolorpixel분석대뇌중동맥(MCA)혈관내경,병이용공식계산MCA적혈류량,대조연구.결과 (1)재이유초성(2DUS)모식하USA측량MCA혈관내경변화분별위SAH건모전(713±10.1)μm화SAH건모후3 d(475±6.4)μm,유명현차이(P<0.05).동시초성다보륵측득기류속분별위(34.67±3.11)cm/s화(55.4±3.81)cm/s(P<0.05).(2)계산혈류량,분별위SAH건모전위(13.86±1.11)×10~(-2) ml/s,SAH건모후3 d위(9.19±0.71)×10~(-2) ml/s;이자차이현저(P<0.01).결론 USA가용우뇌혈류량측량,대SAH후혈관경련급뇌혈류변화구유일정적진단개치.
Objective To research the diameter,flow rate,blood flow of middle cerebral artery(MCA) in vasospasm using ultrasound angiography(USA) and provide a method of measuring diameter and blood flow accurately and successively without wound.Methods Ultrasound angiography(USA) was performed on 18 adult rabbits with subarachnoid hemorrhage(SAH) which were subjected to craniotomy.The inner diameter and flow rate before and after modeling were measured by the USA.The USA images of MCA inner diameter were measured by software Getcolorpixels.The blood flow was calculated using the formula FS =π×(R/2)~2 × V.The MCA parameters were analyzed.Results There was statistically significant difference between before and after SAH using ultrasound angiography.The inner diameter before SAH was 713±15μm and the inner diameter after SAH was 475±6.4μm(P<0.05).The flow rate before SAH was 34.67±3.11cm/s and the flow rate after SAH was 55.4±3.81 cm/s(P<0.05).The blood flow of the two teams were (13.86±1.11)×10~(-2)ml/s and(9.19±0.71)×10~(-2)ml/s respectively(P<0.01).Conclusions USA combining Doppler supersound can calculate MCA blood flow accurately.The method which is simple and reliable has certain diagnostic value.
