为研究运用模糊数学原理预测股骨颈骨折(Fracture of the Femoral NeckFFN)后股骨头缺血性坏死(Avascular Necrosis of the femoral Head ANFH)的方法及其可靠性,将FFN后并发ANFH的相关因素归结为9个因素、28个水平;将并发ANFH的可能性划分为5个等级;对近10年来文献中有关记载进行模糊统计,建立单因素评价集合;采用德尔斐氏法确定各因素重要程度系数,进行模糊初始评判和等级评价,并对45例患者进行了回顾性验证。结果显示被评判为可能性极大、很大、有可能、很小、极小的病例分别为3例、5例、8例、10例、19例,各级别并发ANFH者分别为3例、5例、2例、1例、0例,各等级间X2验P<0.01。表明该方法运算简便,结果可靠,对FFN的诊疗和FFN后并发ANFH的防治有重要的临床意义。

Objective:To study the method and the reliability for predictingthe Ischemic Necrosis of Femoral Head(INFH) after the Fracture of Femoral Neck(FFN) with fuzzy mathematics.Methods:all the factors related to INFH after the fracture of femoral neck can be grouped into 9 kinds of factors(28 levels),and all the possibilities of the complications of INFH can be divided into five classes.The related information of the literatures during recent 10 years was counted with fuzzy mathematics,an evaluating subset of single factor is formed and the important level coefficient is determined with Delphi's method,the primary evaluation and classified evaluation are made with fuzzy mathematics.Results:With this method,45 cases were tested.The complications occurred most possibly,more possibly,possibly,less possibly,least possibly respectively in 3,5,8,10,19 cases,the patients with the INFH of every class occurred in 3 cases,5 cases,2 case,1 case,0case respectively,and each class has been compared with χ2 test with the result of P<0.01.Conclusion:This method is convenient and believable,also is important in clinic for diagnosing and curing FFN to prevent INFH after FFN.
