目的 探讨心脏病合并妊娠的母婴风险及其防治措施.方法 收集北京安贞医院妇产科2002年1月-2009年6月223例心脏病合并妊娠住院产妇的临床资料,进行回顾性分析.结果 心脏病合并妊娠孕产妇占同期全部孕产妇的3.91%,以先天性心脏病最常见;心律失常组较其他类型心脏病患者心力衰竭发生例数少、孕周时间长、新生儿体质量重,差异有统计学意义(P<0.05).孕期规范产检组妊娠结局明显优于未规范产检组,差异有统计学意义(P<0.05);心脏手术治疗组患者母儿预后良好.结论 心脏病合并妊娠属高危妊娠,孕前咨询、孕期规范产检及心脏手术治疗可有效降低女性心脏病患者的妊娠及分娩的风险.

Objective To investigate the clinical features and protective strategies of heart disease in pregnancy.Methods The clinical data of 223 pregnant women with heart diseases who were admitted in Beijing Anzhen Hospital from January 2002 to June 2009 were retrospectively analyzedResults The incidence of heart diseases (mainly congenital heart disease) were 3.91% among the pregnant women..Compared with other types of heart diseases,arrhythmia is associated with significantly lower incidence of heart failure,longer gestational period,and higher neonatal body weight (all P<.05).The pregnancy outcomes were significantly better in women who received regular check-ups than those who did not receive such service (P<0.05).The prognoses of mothers and neonates in patients who had received cardiac surgeries were good.Conclusion Heart disease in pregnancy is a high-risk condition.Pre-pregnancy counseling,regular check-ups during pregnancy,and cardiac surgery are helpful to reduce the risks of heart diseases in pregnancy.
