目的 探讨造影指导下穿刺腋静脉植入起搏器电极导线的方法.方法 2009年5月至2010年4月,共145例患者采用造影指导下穿刺腋静脉植入电极导线.经前臂静脉注射造影剂,记录腋静脉影像,以此为指导,行腋静脉穿刺.结果 穿刺腋静脉植入起搏器电极导线成功率为91.6%,穿刺成功组与穿刺不成功组平均年龄、性别比例差异无统计学意义,1例(0.7%)患者出现气胸,1例(0.7%)因误穿腋动脉并插入扩张管及外鞘导致皮下广泛出血,无电极导线磨损、断裂.结论 造影指导下穿刺腋静脉植入起搏器电极导线安全、有效,是起搏器电极导线植入的重要方法.

목적 탐토조영지도하천자액정맥식입기박기전겁도선적방법.방법 2009년5월지2010년4월,공145례환자채용조영지도하천자액정맥식입전겁도선.경전비정맥주사조영제,기록액정맥영상,이차위지도,행액정맥천자.결과 천자액정맥식입기박기전겁도선성공솔위91.6%,천자성공조여천자불성공조평균년령、성별비례차이무통계학의의,1례(0.7%)환자출현기흉,1례(0.7%)인오천액동맥병삽입확장관급외초도치피하엄범출혈,무전겁도선마손、단렬.결론 조영지도하천자액정맥식입기박기전겁도선안전、유효,시기박기전겁도선식입적중요방법.
Objective To evaluate the safety and efficacy of placement of pacemaker leads using the axillary vein guided by contrast venography. Methods Pacemaker leads were placed using axillary vein guided by contrast venography in 145 patients during May 2009 to April 2010. Contrast dye was injected via a peripheral vein for recording venography image of axillary vein, and the axillary vein was punctured guided by the venography image. Results The success rate of placement of pacemaker leads through axillary vein guided by contrast venography was 91.6%. There was no significant difference between the success group and no success group in average age and sexual ratio. One patient(0. 7% )developed insignificant pneumatothorax,one patient (0. 7% )developed significant subcutaneous hemorrhage caused by puncturing into artery by mistake. There was no patient developed lead abrasion or fracture. Conclusion Placement of pacemaker leads using the axillary vein guided by contrast venography was safe and effective. It' s an important method for placement of pacemaker leads.
