目的 评价尿Cyfra21-1和B超联合检查在膀胱移行细胞癌术后监测中的作用,探讨替代膀胱镜检查的可行性. 方法 对67例膀胱移行细胞癌患者术后定期行尿Cyfra21-1、B超和膀胱镜检查.电化学发光免疫分析(ECLIA)技术测定尿Cyfra21-1.以膀胱镜活检病理结果 为标准,评估尿Cyfra21-1和B超联合检查对膀胱移行细胞癌术后监测的意义.结果 67例膀胱移行细胞癌患者术后34个月内膀胱镜活检发现21例复发;尿Cyfra21-1检测阳性23例;B超检测阳性19例;尿Cyfra21-1和B超联合检查阳性26例,漏诊1例,漏诊率为4.76%.尿Cyfra21-1和B超联合检查与膀胱镜检查比较,经χ2检验无显著性差异(p>0.05).结论 尿Cyfra21-1和B超联合检查可以部分替代膀胱移行细胞癌患者术后的膀胱镜检查.

Objectives To investigate the role of routine follow-up urine cyfra21-1 detecting combined with ultrasonography in detection of recurrence in postoperative bladder transitional cell carcinoma patients.Methods Sixty seven patients under surveillance after reserved bladder operations were recruited from February 2004 to December 2006. Standard ECLIA was used to determine the concentration of urine cyfra21-1. Ultrasonography and cystoscopy were all performed several days thereafter. Cystoscopic biopsy findings were the golden standard for the detection of recurrence of bladder cancer.Results Twenty one patients were detected recurrent tumors by cystoscopic biopsy(31.3%). While 23 patients were positive by urine cyfra21-1 detecting, and 19 cases positive by ultrasonography. Combining urine cyfra21-1 measuring with ultrasonography, the positive cases rose to 26(38.8%), and only one case (4.76%) was omitted.Conclusions Routine follow-up cystoscopy may be partially replaced by urine cyfra21-1 detecting combined with ultrasonography to monitor the recurrence of bladder transitional cell carcinoma.
