目的 分析浅表部位脂肪瘤的声像图特点,探讨超声对其诊断价值.方法 超声检查620例浅表部位脂肪瘤患者,观察肿块数量、部位、形态、大小、边界、内部回声、毗邻关系、距皮肤深度、是否在脂肪层内等,同时观察包块内部及周边的血流分布,分析肿块的超声表现与病理组织学分型的相关性.结果 典型的脂肪瘤380例,超声表现为边界清楚,包膜高回声,肿块内部呈低回声,与周围脂肪回声相似,间有纤细的条纹状高回声分隔;病理组织学表现为大量成熟的脂肪细胞.高回声脂肪瘤175例,超声表现为肿块呈高回声,明显高于周边脂肪组织回声,分界清楚,但无包膜,内部回声较均匀;镜下可见脂肪细胞及毛细血管和纤维组织.非典型脂肪瘤65例.结论 浅表脂肪瘤的超声分型,有助于其诊断.
目的 分析淺錶部位脂肪瘤的聲像圖特點,探討超聲對其診斷價值.方法 超聲檢查620例淺錶部位脂肪瘤患者,觀察腫塊數量、部位、形態、大小、邊界、內部迴聲、毗鄰關繫、距皮膚深度、是否在脂肪層內等,同時觀察包塊內部及週邊的血流分佈,分析腫塊的超聲錶現與病理組織學分型的相關性.結果 典型的脂肪瘤380例,超聲錶現為邊界清楚,包膜高迴聲,腫塊內部呈低迴聲,與週圍脂肪迴聲相似,間有纖細的條紋狀高迴聲分隔;病理組織學錶現為大量成熟的脂肪細胞.高迴聲脂肪瘤175例,超聲錶現為腫塊呈高迴聲,明顯高于週邊脂肪組織迴聲,分界清楚,但無包膜,內部迴聲較均勻;鏡下可見脂肪細胞及毛細血管和纖維組織.非典型脂肪瘤65例.結論 淺錶脂肪瘤的超聲分型,有助于其診斷.
목적 분석천표부위지방류적성상도특점,탐토초성대기진단개치.방법 초성검사620례천표부위지방류환자,관찰종괴수량、부위、형태、대소、변계、내부회성、비린관계、거피부심도、시부재지방층내등,동시관찰포괴내부급주변적혈류분포,분석종괴적초성표현여병리조직학분형적상관성.결과 전형적지방류380례,초성표현위변계청초,포막고회성,종괴내부정저회성,여주위지방회성상사,간유섬세적조문상고회성분격;병리조직학표현위대량성숙적지방세포.고회성지방류175례,초성표현위종괴정고회성,명현고우주변지방조직회성,분계청초,단무포막,내부회성교균균;경하가견지방세포급모세혈관화섬유조직.비전형지방류65례.결론 천표지방류적초성분형,유조우기진단.
Objective To analyze the sonogram features of superficial lipoma, and to explore the diagnostic value by ultrasound. Methods Six hundred and twenty patients with superficial lipoma were examined by ultrasonography. Their quantity, position, shape, size, border, internal echo, adjacent relations, depth from the skin and whether in fat layer were observed. And the condition of masses interior and blood flow distribution were observed at the same time. The correlation of ultrasonic appearance and pathologic histology typing of the masses were analyzed. Results Three hundred and eighty cases of typical lipoma were confirmed by ultrasonography, their appearance was clear boundary with peplos high level echo, low level echo in masses similar to the surrounding fat echo, and slender barred high level separated light pattern.The appearance of pathologic histology was abundant ripe fat cells. There were 175 cases of high level echo lipoma, the ultrasonic appearance was high level echo masses which was significant higher than surrounding fat echo, clear boundary without peplos. Internal echo was uniform. Pathologic histology showed fat cells, capillaries and fibrous tissue,the rest were atypical lipoma. Conclusion The ultrasonic appearance of superficial lipoma has some characteristics. Ultrasonic typing of lipoma has important value in imaging basis and diagnosis of superficial lipoma.
