目的观察谷氨酰胺二肽对Wistar大鼠短肠模型血清蛋白和肝功能的影响,及其减轻脂肪乳剂引起肝损害的作用机理.方法32只Wistar雄性大鼠随机分为谷氨酰胺二肽组(Gln-TPN组)、标准组(Std-TPN组)、无氮组,观察不同组动物之间血清总蛋白、白蛋白、前白蛋白(PA)、纤维结合蛋白(FN)以及谷丙转氨酶和胆红素的变化.结果Gln-TPN组血清总蛋白、纤维结合蛋白与Std-TPN组相比无明显差别,但高于无氮组(P<0.05),白蛋白、前白蛋白明显高于Std-TPN组和无氮组(P<0.05);谷丙转氨酶、总胆红素和直接胆红素三组之间均有差别,Gln-TPN组最低(P<0.05).结论谷氨酰胺二肽能提高短肠综合征大鼠血清蛋白、前白蛋白的含量,抑制全肠外营养(TPN)引起的肝内淤胆作用,减轻脂肪乳剂对肝功能损害,增强其生存能力.

Purpose To investigate the effects of glutsmine on liver function and serum albumin in rats after massive small bowel resection (SBR). Methods After central venous catheterization, 30 Wistar rats were randomly assigned into one of three groups. The nitrogen-free group of rats (n=10) were given an 75% mid-gut resection, leaving 15 cm of the proximal jejunm and 10cm of the distal ileum. Bowel continuity was restored by an end-to-end jejunoileal anastomosis of interrupted 5-0 silk suture, this group was given no amino and protein, but glucose and intralipid thronght central vena cava. The other two groups underwent the same operation as the first group and were divided into 17 amino acids TPN group (Std-TPN group n=10) and 20 amino acids TPN group (Gln-TPN group n=10). The second group and the third group were isonitrogenous, the all three groups were isocaloric. The body weight,liver faction, nitrogen balance, serum prealbumin and fihronectin were measured. Resalts The results showed that all variable in 20AA group and 17AA group were better than nitrogen-free group (P<0.05). The 20AA group had better body weight and nitrogen balance than 17AA group. The serum prealbumin in 20AA group was increased compared with 17AA group (P<0.05 ). The fibronectin, the direct reacting bilirubin and total bilirubin in 17AA group was increased compared with 20AA group (P<0.05)However there was no signiticant difference in proterin and fihronectin lends befween 20AA and 17AA groups. Conelasions The results have shown that 20AA TPN group had better nutritional support, which could prevent eholestasis associated with TPN and reduce liver dysfunction.
