目的探讨恶性肿瘤并发脊髓压迫症( SCCS)的定位诊断及急诊处理。方法对29例恶性肿瘤并发脊髓压迫的临床表现及影像学检查进行分析，根据原发肿瘤的性质及临床表现分别给予急诊放疗、化疗及手术处理。结果 MRI对SCCS的定位诊断阳性率为100.0%；对SCCS引起疼痛、感觉异常、瘫痪的缓解率，急诊放疗分别为91.7%、85.7%、50.0%；急诊化疗则分别为100.0 %、66.7%、22.0%；急诊手术对疼痛的缓解率为100.0%，余无缓解。结论 MRI对SCCS有很高的定位及定性价值，急诊放疗是SCCS原则上首选且显著有效的治疗方法。
목적탐토악성종류병발척수압박증( SCCS)적정위진단급급진처리。방법대29례악성종류병발척수압박적림상표현급영상학검사진행분석，근거원발종류적성질급림상표현분별급여급진방료、화료급수술처리。결과 MRI대SCCS적정위진단양성솔위100.0%；대SCCS인기동통、감각이상、탄탄적완해솔，급진방료분별위91.7%、85.7%、50.0%；급진화료칙분별위100.0 %、66.7%、22.0%；급진수술대동통적완해솔위100.0%，여무완해。결론 MRI대SCCS유흔고적정위급정성개치，급진방료시SCCS원칙상수선차현저유효적치료방법。
Objective　To explore the niveau dignosis and emer gent management on spinal cord compression sygndrome (SCCS) due to malignant tu mours.Methods　Clinical manifestation and imaging scaning of 29 cases of SCCS were analysed.According the characteristics of primary malignant tumors and clinical presentation,emergent radiotherapy and chemotherapy and oper ative management were carried out individually.Results　The accu r acy of MRI was 100.0% in niveau diagnosis of SCCS;The remission rate for pain,nu mbness and paralysis was 91.7%,58.7% and 50% respectively in 22 cases of SCCS u nd ergoing emergent radiotherapy,while it was 100.0%,66.7% and 22.0% respective l y in 5 case of SCCS undergoing emergent chemotherapy.Conclusions　MRI has significantly value in niveau diagnosis of SCCS.Emergent radiotherapy is significantly effective and is the first choice in treating SCCS.
