目的:探讨汕头地区人群HLA-DRB1等位基因分布,及其对风湿性关节炎(RA)易感性及病情和预后估计的意义。方法:用序列特异性引物基因扩增法(PCR-SSP)对117例RA、38例SLE和100例正常人进行DRB1等位基因鉴定;用序列特异性寡核苷酸探针(SSOP)斑点杂交进行DRB1*04亚型鉴定。结果:RA患者DRB1*04频率显著高于正常组(49.6%对18.0%),且以DRB1*0405为主要亚型(62.1%对27.8%)。DRB1*04阳性组RA较阴性组病程长、发病年龄轻(P<0.05),两组间各种自身抗体阳性率无显著性差异(P>0.05)。结论:汕头地区人群RA和DRB1* 04显著相关,其主要亚型为DRB1*0405。DRB1等位基因型别可作为估计RA病情及预后的指标。

목적:탐토산두지구인군HLA-DRB1등위기인분포,급기대풍습성관절염(RA)역감성급병정화예후고계적의의。방법:용서렬특이성인물기인확증법(PCR-SSP)대117례RA、38례SLE화100례정상인진행DRB1등위기인감정;용서렬특이성과핵감산탐침(SSOP)반점잡교진행DRB1*04아형감정。결과:RA환자DRB1*04빈솔현저고우정상조(49.6%대18.0%),차이DRB1*0405위주요아형(62.1%대27.8%)。DRB1*04양성조RA교음성조병정장、발병년령경(P<0.05),량조간각충자신항체양성솔무현저성차이(P>0.05)。결론:산두지구인군RA화DRB1* 04현저상관,기주요아형위DRB1*0405。DRB1등위기인형별가작위고계RA병정급예후적지표。
Objective:To investigate the relationship of HLA-DRB1 alleleswith the susceptibility and clinical features of rheumatoid arthritis(RA)in Shantou.Methods:One hundred and seventeen cases of RA were studied;the control group included 38 cases of systemic lupus erythematosus(SLE)and 100 healthy.DRB1 was typing by DNA amplification with sequence-specific primers(SSP).DRB1*04 subtypes were determined by hybridization with sequence-specific oligonucleotides probes(SSOP).Results:The prevalence rate of DRB1* 04 in RA was higher than that in the control group(49.6% vs 18.0%).The DRB1*0405 accounted for 62.1% of DRB1* 04 positive RA and 27.8% of positive healthy.Compared with DRB1*04 negative RA,the disease duration of positive ones was longer and the onset age was younger.No significant difference was found in the incidence rates of RF,ANA,APF,AKA,anti-RA33 antibodies,anti-Sa antibodies and anti-type II collagen antibodies between two groups.Conclusion:HLA-DRB1* 04 alleles and DRB1*0405 are significantly associated with RA susceptibility in Shantou.DRB1 alleles genotyping may be useful in evaluating the disease severity and prognosis of RA patients.
