目的:探讨非风湿性心房颤动患者经药物复律后左房收缩功能的改变。方法:观察了23例患者药物复律第1、8、15、30及60天时的二尖瓣血流变化并与20例正常人作对比研究。结果:患者复律后2周内的左室舒张晚期二尖瓣血流峰速、速度积分显著增大,此后2周内渐大,而同期的左室舒张早期血流峰速、速度积分减小;与正常人相比,患者1个月、2个月时左室舒张晚期二尖瓣血流仍有显著性不同(P<0.05),但两者间无显著性差异。结论:患者恢复窦性心律后左房收缩功能恢复主要在1个月内逐步进行,且2个月内不能完全恢复,多普勒超声观察有助于指导药物治疗及疗效评价。

목적:탐토비풍습성심방전동환자경약물복률후좌방수축공능적개변。방법:관찰료23례환자약물복률제1、8、15、30급60천시적이첨판혈류변화병여20례정상인작대비연구。결과:환자복률후2주내적좌실서장만기이첨판혈류봉속、속도적분현저증대,차후2주내점대,이동기적좌실서장조기혈류봉속、속도적분감소;여정상인상비,환자1개월、2개월시좌실서장만기이첨판혈류잉유현저성불동(P<0.05),단량자간무현저성차이。결론:환자회복두성심률후좌방수축공능회복주요재1개월내축보진행,차2개월내불능완전회복,다보륵초성관찰유조우지도약물치료급료효평개。
Objective: To learn the characteristic of recovery of left atrial systolic function after pharmacological conversion to sinus rhythm in patients with nonrheumatic atrial fibrillation. Method :Transmitral Doppler patterns in 20 normal people were examined and those in 23 patients with nonrheumatic atrial fibrillation were followed up within the first day and on day 8,15,30,60 after cardiovarsion. Result:There was a significant increase in the peak A-wave velocity during the first two weeks and a gradual increase within the later two weeks while the peak E-wave velocity desended. Compared with those in the normal controls,the peak A-wave velocity was lower on day 30 and day 60 respectably. But there was no difference between them. Conclusion: The restoration of atrial mechanical function after pharmacological conversion during the first month was gradual but significant. It was not complete within two months. It may be important for us to guide and evaluate treatment clinic by study this change from echocardiography.
