目的:探讨新生儿病理性黄疸心肌酶变化及临床意义。方法:分别于入院时(治疗前)和治疗后(显性黄疸消退后),抽不抗凝血2ml,用全自动生化分析仪测定心肌酶、肝功能、肾功能等10项,部分病人同时作心电图检查.结果:新生儿病理性黄疸40例心肌酶均升高,其中4种酶升高者24例(60%),3种酶升高者16例(40%),新生儿生理性黄疸30例,心肌酶1种或2种升高者5例,两组比较差异有非常显著意义(P<0.01)。病程在7d以内,总胆红素,心肌酶明显高于病程在7d以上者。治疗前后,总胆红素心肌酶比较、差异有非常显著意义(P<0.01)。结论:新生儿病理性黄疸可出现心肌损害,与黄疸出现时间和程度有关。在抗菌基础上,加用VitC.ATP.CoA营养心肌细胞,心肌酶随显性黄疸消退而下降,疗效较好。随访15例病理性黄疸心肌酶未恢复正常者,均在1~2月内心肌酶恢复正常,心肌损害呈可逆性。

목적:탐토신생인병이성황달심기매변화급림상의의。방법:분별우입원시(치료전)화치료후(현성황달소퇴후),추불항응혈2ml,용전자동생화분석의측정심기매、간공능、신공능등10항,부분병인동시작심전도검사.결과:신생인병이성황달40례심기매균승고,기중4충매승고자24례(60%),3충매승고자16례(40%),신생인생이성황달30례,심기매1충혹2충승고자5례,량조비교차이유비상현저의의(P<0.01)。병정재7d이내,총담홍소,심기매명현고우병정재7d이상자。치료전후,총담홍소심기매비교、차이유비상현저의의(P<0.01)。결론:신생인병이성황달가출현심기손해,여황달출현시간화정도유관。재항균기출상,가용VitC.ATP.CoA영양심기세포,심기매수현성황달소퇴이하강,료효교호。수방15례병이성황달심기매미회복정상자,균재1~2월내심기매회복정상,심기손해정가역성。
Objective:To investigate the changes and significance of testing myocardium enzyme to the pathologic jaundice for the newborn .Methods:When the patients go to hospital (before the treatment)and after the tratment,draw 2ml blood without anticoagulant,have a myocardium enzyme test,liver function test,renal function test and so on, and some patients may have electrocardiogram tests.Results:The myocardium enzyme of 40 patients with pathologic jaundice has risen,of whom 24 with 4 kinds of enzyme rising(60%),16 with 3 kinds enzyme rising(40%).Among 30 patients with the physiologic jaundice,5 patients with 1 or 2 kinds of enzyme rising.Compared with the two groups,the difference is very evident and significant (P<0.01).The total bilirubin and the myocardium enzyme of the patients whose course of disease is within 7 days are higher than those whose course of disease is over 7 days.Compare the total bilirubin and the myocardium enzyme before the treatment and after the treatment,the difference is very evident and significant(P<0.01).Conclusion:The newborn with the pathologic jaundice may have their cardiac muscle damaged.It is connected with the degree and the time when jaundice appears.After using the antibiotic,add nutrition myocardium cells with VitC.ATP.CoA,the myocardium enzyme will fall with the jaundice disappearing.The curative effect is good.15 patients whose pathologic jaundice and the myocardium enzyme are unusual all recover normally within 1-2 months.The damage of cardiac muscle is reversible.
