目的 研究支气管动脉化疗及其与支气管腔内放疗联合治疗对非小细胞肺癌细胞凋亡的影响。方法 抽取1996~1998年间在我院接受手术治疗的55例非小细胞肺癌。其中单纯手术切除的20例为A组(对照组);术前2周接受过1~2次支气管动脉化疗的20例为B组;术前2~3周接受过支气管动脉化疗与支气管腔内放疗联合治疗的15例为C组。应用原位DNA断端标记法检测3组标本的癌细胞凋亡水平,以光镜下计数的每100个癌细胞的平均凋亡数(调亡指数)表示。结果 1. A、B、C 3组间凋亡指数两两比较差异均有显著性;2. 3组间鳞癌凋亡指数比较差异有显著性(P<0.05);3. 3组间腺癌凋亡指数比较,A、C两组间有统计学意义(P<0.05),其余两组间均无统计学意义(P>0.05);4. 3组内鳞癌和腺癌间凋亡指数比较均无统计学意义(P>0.05)。结论 支气管动脉化疗及其与支气管腔内放疗的联合治疗均能诱导非小细胞肺癌细胞凋亡,其中以联合治疗的诱导作用较强。

목적 연구지기관동맥화료급기여지기관강내방료연합치료대비소세포폐암세포조망적영향。방법 추취1996~1998년간재아원접수수술치료적55례비소세포폐암。기중단순수술절제적20례위A조(대조조);술전2주접수과1~2차지기관동맥화료적20례위B조;술전2~3주접수과지기관동맥화료여지기관강내방료연합치료적15례위C조。응용원위DNA단단표기법검측3조표본적암세포조망수평,이광경하계수적매100개암세포적평균조망수(조망지수)표시。결과 1. A、B、C 3조간조망지수량량비교차이균유현저성;2. 3조간린암조망지수비교차이유현저성(P<0.05);3. 3조간선암조망지수비교,A、C량조간유통계학의의(P<0.05),기여량조간균무통계학의의(P>0.05);4. 3조내린암화선암간조망지수비교균무통계학의의(P>0.05)。결론 지기관동맥화료급기여지기관강내방료적연합치료균능유도비소세포폐암세포조망,기중이연합치료적유도작용교강。
Objective To study the apoptosis of non-small cell lung cancer (NSCLC) cells after bronchial artery infusion chemotherapy (BAI) alone or/BAI combined with intrabronchial brachytherapy (IBB). Methods The apoptotic level of lung cancer cell was detected in 55 cases of NSCLC samples by TUNEL. 20 of which were treated with surgical resection alone (group A), 20 of which were treated with BAI alone before surgical resection (group B), and 15 of which were treated with BAI combined with IBB before surgical resection (group C). Results 1. The apoptotic index (AI) in the three groups was 8.92±2.82%, 10.76±2.18%, 12.43±1.82% respectively with statistical difference (group A vs group B and group B vs group C showed P<0.05, group A vs group C showed P<0.001); 2. AI of squamous carcinoma cells among the three groups showed P<0.05; 3. AI of adenocarcinoma between group A and group C showed P<0.05, and the other comparisons showed P>0.05; 4. The comparisons of AI between squamous carcinoma cell and adenocarcinoma cell in the three groups all showed P>0.05. Conclusions Apoptosis is another important mechanism leading to cell death in NSCLC after BAI alone or/BAI combined with IBB. Apoptosis induced by the latter is higher to BAI alone.
