目的 探讨慢性心力衰竭患者外周血B型脑利钠肽(BNP)、C-反应蛋白(CRP)浓度与超声心动图指标变化之间的关系.方法 采用Trige分析仪检测外周血BNP的浓度,免疫浊度法测定CRP的浓度,超声诊断仪测量左房内径(LAD)、左室舒张末内径(LVEDD)、左室射血分数(LVEF)、左室短轴缩短率(FS)等数值,比较正常对照组(21人),心脏病心功能正常组(33例),慢性心力衰竭组(61例)BNP、CRP浓度变化与临床NYHA心功能分级情况.结果 与正常对照组和心脏病心功能正常组相比较,BNP、CRP、LAD、LVEDD在慢性心力衰竭组均有升高,LVEF和FS均减小,差异有统计学意义(P<0.05).BNP、CRP、LAD、LVEDD分别随心功能分级的升高而升高,呈正相关(P<0.05),LVEF和FS随心功能的升高而降低,呈负相关(P<0.05).结论 慢性心力衰竭患者存在心脏结构的重塑和非特异炎性反应,外周血BNP、CRP浓度升高,且与NYHA心功能分级密切相关.NYHA心功能分级越高,心室结构重塑和非特异性炎症反应也越明显.

목적 탐토만성심력쇠갈환자외주혈B형뇌리납태(BNP)、C-반응단백(CRP)농도여초성심동도지표변화지간적관계.방법 채용Trige분석의검측외주혈BNP적농도,면역탁도법측정CRP적농도,초성진단의측량좌방내경(LAD)、좌실서장말내경(LVEDD)、좌실사혈분수(LVEF)、좌실단축축단솔(FS)등수치,비교정상대조조(21인),심장병심공능정상조(33례),만성심력쇠갈조(61례)BNP、CRP농도변화여림상NYHA심공능분급정황.결과 여정상대조조화심장병심공능정상조상비교,BNP、CRP、LAD、LVEDD재만성심력쇠갈조균유승고,LVEF화FS균감소,차이유통계학의의(P<0.05).BNP、CRP、LAD、LVEDD분별수심공능분급적승고이승고,정정상관(P<0.05),LVEF화FS수심공능적승고이강저,정부상관(P<0.05).결론 만성심력쇠갈환자존재심장결구적중소화비특이염성반응,외주혈BNP、CRP농도승고,차여NYHA심공능분급밀절상관.NYHA심공능분급월고,심실결구중소화비특이성염증반응야월명현.
Objective To explore the correlation of the concentration changes of plasma B-type natriuretic peptide (BNP) and C-reactive protein (CRP) and the parameters measured by echocardiography.Methods The concentrations of BNP and CRP were measured with Triage Analyzer and enzyme-linked immunoassay;the parameters of left atrial diameter (LAD),left ventricle end-diastolic diameter (LVEDD),left ventricle ejection fraction (LVEF) and fraction shortening(FS)were detected with echocardiography.Then the concentration changes of BNP and CRP,as well as grading of cardiac function according to New York heart association class (NYHA) were compared among normal control group (n=21),heart disease with normal cardiac function group (n=33) and chronic heart failure group (n=61).Results The levels of BNP,CRP, LAD,LVEDD in the chronic heart failure group increased,but the levels of LVEF and FS decreased,with significant differences as compared with the heart disease with normal cardiac function group and control group (P<0.05).As the increased grading of cardiac function,the levels of BNP,CRP,LAD,LVEDD increased,with a positive correlation (P<0.05);but the levels of LVEF and FS decreased,with a negative correlation (P<0.05).Conclusion There are non-special inflammatory lesion and ventricular structure remodeling in patients with chronic heart failure,increased concentrations of BNP and CRP in patients with chronic heart failure,which are closely associated with NYHA grading of cardiac function.The more upgraded the NYHA grading,the more severe the non-special inflammatory lesion and ventricular structure remodeling.
