目的 探讨最佳的经口机械通气患者口腔护理频次,达到以最少的口腔护理频次保证经口机械通气患者的口腔清洁、预防VAP的发生的效果.方法 将36例接受经口气管插管机械通气的患者随机分为A、B、C、D 4组各9例,4组均采用0.5%聚维酮碘进行口腔护理,4组进行口腔护理的时间间隔分别为4,6,8,12 h.分别于本次口腔护理后和下一次口腔护理前行口咽部取样进行细菌菌落半定量计数、细菌菌种鉴定.对4组的机械通气性肺炎的发生率进行比较,采用χ2检验.结果 4组细菌数量变化比较差异显著,差异突出表现在间隔6 h和12 h这2个时段.4组的机械通气性肺炎的发生率比较差异不明显.结论 在预防机械性通气性肺炎发生的前提下间隔6 h进行1次口腔护理是最佳频次.

Objective To discuss oral nursing frequency of patients undergoing orally mechanical ventilation in order to ensure the oral cleanness of patients with the least oral nursing frequency and prevent the occurrence of ventilator associated pneumonia (VAP). Methods 36 patients undergoing orally me-chanical ventilation were randomly divided into groupA, B, C and D with 9 cases in each group, which all adopted 0.5% of pevidone iodine solution for oral nursing, time intervals of oral nursing were 4 hours, 6 hours, 8 hours and 12 hours. After the oral care and oral care before the next oropharyngeal samples for se-mi-quantitative bacterial colony counts, bacterial strain identification were obtained. The incidence rate of VAP was also compared and the results underwent χ2 test. Results Significant difference was seen in bacterial colony counts, especially during the time interval of 6 hours and 12 hours. No significant differ-ence was seen in incidence of VAP between the 4 groups. Conclusions The desirable frequency of oral nursing was every 6 hours on the premise of prevention of VAP.
