目的降低膀胱尿路上皮癌术后复发率,评价内生场热化疗联合应用的疗效和安全性.方法对20例膀胱尿路上皮癌术后患者,术后24 h内行丝裂霉素20 mg/20 ml膀胱灌注1次,术后1个月开始在膀胱内注入丝裂霉素20 mg/100 ml,采用仰卧位和俯卧位各10 min后,内生场膀胱区热疗1h,每月1次,每3个月行膀胱镜检查.若1年无复发,改为每2个月1次热化疗,半年复查1次膀胱镜,1年后无复发可选择性膀胱灌注,长期随访,总疗程为2年,丝裂霉素总灌注量360 mg.结果 20例患者中1例失访,余19例均获随访,随访3～32个月,除1例术后9个月复发外,余18例均未见肿瘤复发及转移,复发率为5.26％.结论内生场热化疗联合应用预防膀胱肿瘤术后复发效果满意,安全性好.
목적 강저방광뇨로상피암술후복발솔,평개내생장열화료연합응용적료효화안전성.방법 대20례방광뇨로상피암술후환자,술후24 h내행사렬매소20 mg/20 ml방광관주1차,술후1개월개시재방광내주입사렬매소20 mg/100 ml,채용앙와위화부와위각10 min후,내생장방광구열료1h,매월1차,매3개월행방광경검사.약1년무복발,개위매2개월1차열화료,반년복사1차방광경,1년후무복발가선택성방광관주,장기수방,총료정위2년,사렬매소총관주량360 mg.결과 20례환자중1례실방,여19례균획수방,수방3～32개월,제1례술후9개월복발외,여18례균미견종류복발급전이,복발솔위5.26％.결론 내생장열화료연합응용예방방광종류술후복발효과만의,안전성호.
Objective To evaluate the efficacy and safety of endogenous field thermal chemotherapy for the prevention of recurrence of urothelial carcinoma of bladder after surgery.Methods Twenty cases of urothelial carcinoma of bladder who underwent surgical management received bladder perfusion with mitomycin in 24 hours bostoperatively,and one month postoperatively received bladder perfusion with mitomycim 20 mg,supined aud proned every 10 minutes,and then received endogenous field thermal chemotherapy for one hour,monthly,and received the bladder endoscopy every three months.If no recurrence occured after one year,then changed to bimonthly,and received bladder endoscopy every half a year.Total treatment was for two years,total dose was 360 mg.Results All 20 cases except one were followed up for 3 months to 32 months.One case got recurrence within 9 months.The recurrence rate was 5.26％.No obvious side effect was found in all cases.Conclusions Endogenous field thermal chemotherapy is effective for preventing the recurrence of urothelial carcinoma of the bladder after surgical management.
