目的 探讨泪小管吻合手术技巧,比较两种泪道置管的疗效.方法 回顾分析95例(95眼)下泪小管断裂吻合术,其中直接夹取法取出鼻泪道引线37例,注水擤鼻法取出鼻泪道引线30例,均逆行置入硅胶管;直接置入硬膜外麻醉管28例,采用X2检验比较两种置管方法的疗效.结果 95例泪小管断端均成功找到.注水擤鼻法取出鼻泪道引线优于直接夹取法.逆行硅胶管置入有效率达97.01%(65/67),直接硬膜外导管置人有效率达82.14%(23/28),差异有统计学意义(X2=4.06,P<0.05).结论 泪小管断裂吻合术中,硅胶管逆行置入疗效优于硬膜外导管直接置入.注水擤鼻法能快捷取出鼻泪道引线,顺利逆行置入义管,适合在基层医院开展.

Objective To investigate the operative technique of lacrimal canaliculi laceration anastomosis, and to compare the effect of two different methods of pipe placement into the lacrimal passage. Methods Ninty-five cases of lower lacrimal canaliculi laceration anastomosis were analysed retrospectively. The traction suture in nasolacrimal duct was removed directly in 37 cases. Then the silicon pipe was placed into the nasolacrimal duct. Twenty-eight cases were placed with hard theca anaesthetic pipes directly. The curative effects of the two groups were examined by X2 test. Results There were 95 cases whose breaking ends of lacrimal canaliculi were successfully found. The effect of removing traction suture by blowing nose with water injection was better than removing it directly. Nasolacrimal intubation with silicon pipe showed an effective rate of 97. 01 % (65/67) which was significantly higher than that of palcing hard theca anaesthetic pipe(82. 14%, 23/28 ; X2 =4.06, P < 0.05). Conclusions In lacrimal canaliculi laceration anastomosis, silicon pipe nasolacrimal intubation was better than hard theca anaesthetic pipe. Blow nose with water injection could quickly remove the traction suture in nasolacrimal duct, and could successfully place the alloy tube, which could be performed in primary hospitals.
