目的 观察鞘内注射(intrathecal,i·t)加巴喷丁和吗啡对切口痛大鼠脊髓氨基酸类神经递质的影响.方法 选择鞘内置管成功的雄性SD大鼠40只,随机分为5组,每组8只,分别为假手术组,对照组,加巴喷丁50μg组,吗啡2.5μg组,加巴喷丁50μg加吗啡2.5 μg组.按Yaksh法鞘内置管;按Brennan法制作大鼠足底切口疼痛模型;用机械缩爪反射阈值(mechanical withdrawal threshold,MWT)和热缩爪潜伏期(thermal withdrawal latency,TWL)来确定疼痛行为学变化;应用高效液相色谱法(HPLC)测定脊髓氨基酸类神经递质如谷氨酸(glutamate,Glu)、天门冬氨酸(aspartate,Asp)、甘氨酸(glycine,Gly)、γ-氨基丁酸(γ-aminobutvric acid,GABA)含量的变化.结果 与假手术组比较,对照组大鼠在术后2 h时MWT明显降低,TWL明显缩短(P<0.05),脊髓Glu、Asp含量明显升高(P<0.05);与对照组比较,术前i·t加巴喷丁50μg加吗啡2.5μg在术后2 h的MWT明显增加,TWL明显延长(P<0.05),脊髓Glu、Asp含量明显减少(P<0.05).结论 在大鼠切口痛模型中,鞘内注射加巴喷丁能增强鞘内吗啡的抗伤害作用,其机制可能与脊髓兴奋性氨基酸含量减少有关.

목적 관찰초내주사(intrathecal,i·t)가파분정화마배대절구통대서척수안기산류신경체질적영향.방법 선택초내치관성공적웅성SD대서40지,수궤분위5조,매조8지,분별위가수술조,대조조,가파분정50μg조,마배2.5μg조,가파분정50μg가마배2.5 μg조.안Yaksh법초내치관;안Brennan법제작대서족저절구동통모형;용궤계축조반사역치(mechanical withdrawal threshold,MWT)화열축조잠복기(thermal withdrawal latency,TWL)래학정동통행위학변화;응용고효액상색보법(HPLC)측정척수안기산류신경체질여곡안산(glutamate,Glu)、천문동안산(aspartate,Asp)、감안산(glycine,Gly)、γ-안기정산(γ-aminobutvric acid,GABA)함량적변화.결과 여가수술조비교,대조조대서재술후2 h시MWT명현강저,TWL명현축단(P<0.05),척수Glu、Asp함량명현승고(P<0.05);여대조조비교,술전i·t가파분정50μg가마배2.5μg재술후2 h적MWT명현증가,TWL명현연장(P<0.05),척수Glu、Asp함량명현감소(P<0.05).결론 재대서절구통모형중,초내주사가파분정능증강초내마배적항상해작용,기궤제가능여척수흥강성안기산함량감소유관.
Objective To study the effects of intrathecally administering gabapentin and/or morphine on the amino acid neurotransmitters in spinal cord of rat incision pain models.Methods Forty male SD rats were randomly divided into five groups (n=8) and were intrathecally inserted catheter.Paw incision models were made 5d later.Gabapentin (50 μg,group Ⅲ) or morphine(2.5 μg,group Ⅳ) or both (group Ⅴ) were intrathecally injected 30 min before incision.Another two groups were regarded as shame (group Ⅰ) or control (group Ⅱ).Pain behavior was determined 2 h later using mechanical withdrawal threshold (MWT) and thermal withdrawal latency (TWL).The content of four amino acids including glutamate,aspartate,glycine and γ-aminobutyric acid in rat spinal cord were detected.Results MWT and TWL decreased and the content of glutamate and aspartate in spinal cord increased significantly 2 h after incision(P<0.05)in groups Ⅱ,Ⅲ and Ⅳ (without statistically differences among three groups),but not in group Ⅴ,compared to them in group Ⅰ.There were no differences in the content of glycine and γ-aminobutyric acid in rat spinal cord 2 h after incision among four groups.Conclusion Intrathecal gabapentin significantly enhances the antinociceptive effect of intrathecal morphine in rat incision pain model,which may owe to the decreased level of excitatory amino acids in spinal cord.
